食管鳞癌患者EGFR蛋白表达及基因扩增特点的临床病理学研究

摘要 目的探讨食管鳞癌患者EGFR蛋白表达及基因扩增特点的临床病理学意义。方法采用免疫组织化学(IHC)法和荧光原位杂交(FISH)法检测患者食管鳞癌组织中EGFR蛋白表达和基因扩增情况,分析其临床病理特点。结果患者食管鳞癌组织EGFR蛋白过表达率为71.83%,不同性别组、不同年龄组、不同肿瘤分化程度组、有无淋巴结转移组以及不同浸润深度组之间比较,EGFR蛋白表达差异无统计学意义(P>0.05)。不同性别、不同年龄、不同浸润深度患者之间比较,EGFR基因扩增差异无统计学意义(P>0.05)。不同肿瘤分化程度和有无淋巴结转移患者之间比较,EGFR基因扩增差异有统计学意义(P<0.05)。EGFR蛋白表达与基因扩增相关,尤其是高表达(2/3+)与基因扩增密切相关。EGFR基因扩增的患者生存时间短于EGFR无扩增的患者,EGFR基因扩增是食管鳞癌患者不良预后的独立危险因素(P<0.05)。结论EGFR基因参与食管鳞癌的发生、发展及转移,蛋白表达及基因扩增的联合检测,可作为判断食管癌预后及指导靶向药物治疗的重要方法。 Objective To investigate the clinicopathological significance of EGFR protein expression and gene amplification in patients with esophageal squamous cell carcinoma.Methods Immunohistochemistry(IHC)and fluorescence in situ hybridization(FISH)were used to detect EGFR protein expression and gene amplification in esophageal squamous cell carcinoma(ESCC),and the clinicopathological characteristics were analyzed.Results The overexpression rate of EGFR protein in esophageal squamous cell carcinoma was 71.83%.There was no significant difference in EGFR protein expression among different gender groups,age groups,tumor differentiation degree groups,lymph node metastasis groups and different invasion depth groups(P>0.05).There was no significant difference in EGFR gene amplification among patients with different gender,age and depth of invasion(P>0.05).There were significant differences in EGFR gene amplification among patients with different tumor differentiation and lymph node metastasis(P<0.05).EGFR protein expression is related to gene amplification,especially high expression(2/3+)is closely related to gene amplification.The survival time of patients with EGFR gene amplification was shorter than that of patients without EGFR amplification.EGFR gene amplification was an independent risk factor of poor prognosis in patients with esophageal squamous cell carcinoma(P<0.05).Conclusion EGFR gene is involved in the occurrence,development and metastasis of esophageal squamous cell carcinoma,and the combined detection of protein expression and gene amplification can be used as an important method to judge the prognosis of esophageal cancer and guide targeted drug therapy.
