儿童难治性肺炎支原体肺炎炎症反应特征分析 被引量:7

摘要 目的探讨难治性肺炎支原体肺炎的免疫与炎症反应的变化及与病情相关性,为早期及时制订合理治疗方案提供临床依据。方法对我院2014年6月-2018年12月的难治性肺炎支原体肺炎(RMPP)儿童40例(实验组)、轻症肺炎支原体肺炎(MPP)儿童46例(对照组)的急性期血清中CD4+T细胞、CD8^+T细胞、NK细胞及CD4+/CD8+比值、免疫球蛋白(IgA、IgM、IgG、IgE)、IL-2、IL-6、IL-8、IL-10进行检测。结果46例轻症MPP组的患儿中,男25例,女21例,平均年龄(5.21±1.64)岁;40例RMPP患儿中,男12例,女28例,平均(7.19±1.85)岁。RMPP组女性患儿比例明显增多,两组间在性别差异上存在统计学意义[T(χ^2)=5.174,P<0.05]。轻症MPP组年龄明显小于RMPP组[T(χ^2)=5.27,P<0.05],轻症MPP组与RMPP组gM、IgE、IL-8、IL-6两组比较,RMPP较轻症MPP的有明显升高(P<0.05)。CD4^+T、CD4^+/CD8^+、NK细胞在RMPP中有明显降低(P<0.05)。结论RMPP多发生在学龄期儿童,免疫与炎症指标的综合系统分析有助早期识别RMPP。 Objective To investigate the changes in immune and inflammatory responses of refractory Mycoplasma pneumoniae pneumonia and their correlation with the disease,so as to provide clinical evidence for early and timely formulation of a reasonable treatment plan.Methods Forty children with refractory mycoplasma pneumonia(RMPP)(experimental group)and 46 children with mild mycoplasma pneumonia(MPP)(control group)who were hospitalized in our hospital from June 2014 to December 2018 were selected.In their acute phase serum CD4+T cells,CD8+T cells,NK cells and CD4+/CD8+ratio,immunoglobulin(IgA,IgM,IgG,IgE),IL-2,IL-6,IL-8,IL-10 was tested.Results Among the 46 children in the mild MPP group,25 were males and 21 were females,with an average age of 5.21±1.64 years;among 40 children with RMPP,12 were males and 28 were females,with an average of(7.19±1.85)years.The proportion of female children in the RMPP group increased significantly,and there was a statistically significant difference in gender between the two groups[T(χ^2)value=5.174,P<0.05].The age of the mild MPP group was significantly smaller than that of the RMPP group[T(χ^2)value=5.27,P<0.05].Compared with the two groups of gM,IgE,IL-8 and IL-6 in the RMPP group,the mild MPP group showed that RMPP was lower than the RMPP group.There was a significant increase(P<0.05).CD4^+T,CD4^+/CD8^+,NK cells were significantly reduced in RMPP(P<0.05).Conclusion RMPP mostly occurs in school-age children.The comprehensive systematic analysis of immune and inflammatory indicators can help identify RMPP early.
