摘要
目的比较两种内镜辅助下鼻空肠营养管置入方式的有效性及安全性。方法回顾性分析2017年12月至2019年12月中国人民解放军总医院消化内科行内镜下空肠营养管置入术的310例患者,其中内镜下钳夹推送置入鼻空肠营养管265例,鼻内镜下导丝引导置入鼻空肠营养管45例。结果310例患者均在无麻醉支持的情况下进行内镜下鼻空肠营养管置入,300例成功,10例失败,其中内镜下钳夹推送置管成功率为97.7%(259/265),鼻胃镜下导丝引导置管成功率为91.1%(41/45),二者之间具有统计学差异(P<0.05)。两组的中位操作时间分别为25(18,34)min和58(27.5,83)min,二者之间具有统计学差异(P<0.01);两组的中位花费分别为1659.23(1659.23,1670.53)元和2829.62(1550.19,3480.24)元,两者之间具有统计学差异(P<0.01)。结论内镜下钳夹推送法,可在直视下操作,操作简单,成功率高,耗费时间短,患者痛苦小,建议作为内镜下放置鼻空肠营养管的首选方法。
Objective To compare the efficacy and safety of two different methods of endoscopic assisted nasal jejunum feeding.Methods The data of 310 patients underwent endoscopic nasal jejunum feeding tube implantation from December 2017 to December2019 were reviewed in this study,in which endoscopic clamp was placed in 265 cases and guide wire was applied in 45 cases for nasal jejunum feeding tube placement in the Department of Gastroenterology,Chinese PLA general hospital.Results All 310 patients underwent endoscopic nasal jejunal nutrition tube placement without anesthesia support.300 cases were successful and 10 failed.The success rate of endoscopic clamp was 97.7%(259/265),the success rate of guide wire was 91.1%(41/45),there was a significant difference between the two(P<0.05),and The average operation time of the two groups of patients was respectively was 25(18,34)min and 58(27.5,83)min,there is a significant difference between the two(P<0.01);The average cost of the two groups of patients was 1659.23(1659.23,1670.53)$and 2829.62(1550.19,3480.24)$,there is a significant difference between the two(P<0.01).Conclusion The endoscopicclamp can be operated under direct vision,with simple operation,high success rate,short timeconsuming,and less pain for patients.It is recommended as the first method for placing nasal jejunal nutrition tube under endoscope.
