摘要
目的探究辨证施膳结合健康教育对幽门螺杆菌(Helicobactor pylori,HP)阳性慢性萎缩性胃炎根治HP治疗的应用效果。方法选取2018年5月—2019年3月在广州中医药大学深圳医院(福田)就诊行胃镜、病理学的幽门螺杆菌阳性的慢性萎缩性胃炎患者70例为研究对象,采用随机数字表法随机分为干预组和对照组各35例,两组均予常规四联(埃索美拉唑镁肠溶片+枸橼酸铋钾颗粒+阿莫西林胶囊+克拉霉素缓释片)根治HP治疗,对照组给予常规护理,干预组根据辨证分型在健康教育的基础上辨证施膳,6周后比较两组的HP根除率,消化不良、反流、排便、全身状况、心理、社会功能各维度生存质量积分及总积分。结果干预组治疗后HP根除率明显高于对照组,组间比较差异有统计学意义(P <0.05);生存质量积分方面,治疗前两组在消化不良、排便、全身状况、心理、社会功能各维度的积分及总积分方面比较均无明显差异(P> 0.05),治疗后两组各维度的积分及总积分方面均较治疗前明显改善(P <0.05),且干预组在消化不良、全身状况、心理、社会功能维度的积分及总积分方面疗效显著优于对照组(P <0.05)。结论辨证施膳结合健康教育能提高HP根除率及生存质量积分,在消化不良、全身状况、心理、社会功能4个维度改善明显。
Objective To study the effect of using dialectical diet combined with health education in eradicating Helicobacter pylori in chronic atrophic gastritis patients with Helicobacter pylori positive. Methods Choose 70 chronic atrophic gastritis patients with Helicobacter pylori positive who was underwent endoscopy and pathological examination in Shenzhen Hospital of Guangzhou TCM University( Futian) from May 2018 to March 2019 as subjects in this study,separated them as 35 cases in intervented group and 35 cases in control group randomly by using random Number table method,conventional nursing care was applied in control group while dialectical diet combined with health education added in intervented group when both groups using Quadruple eradicating HP therapy( Esomeprazole Magnesium Enteric-coated Tablets + bismuth potassium citrate granules + Amoxicillin capsules + Clarithromycin Sustained Release Tablets),and comparing HP eradicated rate,the indigestion,reflux,defecation,general condition,psychology,social function and total score between both groups after treatment. Results HP eradicated rate in intervented group washigher significantly than control group,with a significant difference between groups( P < 0. 05). In life quality,before treatment the indigestion,reflux,defecation,general condition,psychology,social function and total score in two groups were with a insignificant difference( P > 0. 05). Each items were significantly improved after treatment,besides the items of indigestion,general condition,psychology,social function and total score in intervented group were higher significantly than those in control group( P < 0. 05). Conclusion Dialectical diet combined with health education in eradicating Helicobacter pylori in chronic atrophic gastritis patients with Helicobacter pylori positive can improve HP eradicated rate and life quality scale,especially in four items including indigestion,general condition,psychology,social function.
