摘要
目的分析A型主动脉夹层(TAAD)行孙氏手术的临床疗效及对血清IL-8、cTnT水平的影响。方法选定牡丹江心血管病医院2016年8月—2019年8月收治的60例TAAD患者,以双盲随机抽样法分组(每组30例),参照组给予部分主动脉弓替换手术治疗,实验组给予孙氏手术治疗,对比两组手术指标、血清IL-8、cTnT水平、并发症发生率、病死率。结果实验组手术时间、阻断主动脉时间与参照组比较,P>0.05。实验组选择性脑灌注时间以及ICU住院时间均显著比参照组短,实验组术后3d血清IL-8、cTnT水平均显著比参照组低,实验组并发症发生率(3.33%)、病死率(0.00%)均显著比参照组(26.67%、13.33%)低,差异均具有统计学意义(P<0.05)。结论孙氏手术可有效下调TAAD患者血清IL-8、cTnT高表达状态,降低并发症发生率、病死率,且住院时间较短,值得借鉴。
Objective To analyze the clinical effect of TAAD(type A aortic dissection)on Sun’s surgery and its effect on se rum IL-8 and cTnT levels.Methods 60 TAAD patients admitted to the hospital from August,2016 to August,2019 were selected and grouped by double-blind random sampling(30 cases in each group).The reference group was given partial aortic arch replace ment surgery,and the experimental group was given Sun’s surgery.The surgical index,serum IL-8 and cTnT level,complica tion rate and mortality of the two groups were compared.Results Compared with the reference group,the operation time and the aortic block time in the experimental group were P>0.05.The selective cerebral perfusion time and ICU hospitalization time in the experimental group were significantly shorter than those in the reference group.The serum IL-8 and cTnT levels in the experi mental group were significantly lower than those in the reference group after 3 days.The complication rate(3.33%)and mortality(0)in the experimental group were significantly lowe than the reference group(26.67%and 13.33%),P<0.05(the differences were statis tically significant).Conclusion Sun’s surgery can effectively down-regulate serum high levels of IL-8 and cTnT in patients with TA AD,reduce the incidence of complications and mortality,and the length of hospital stay is short,which is worth learning from.
