摘要
目的探讨超微血管三维立体成像(Smart 3D SMI)鉴别乳腺肿物良恶性的价值。方法选取2019年2月至2020年4月于大连医科大学附属第一医院病理确诊的106例患者(114个乳腺肿物),均行Smart 3D SMI检测,观察肿瘤微血管的三维形态,与病理结果对比,评价其诊断作用。结果114个乳腺肿物中良性肿物60个,Smart 3D SMI显示血管形态以线型(14个)、树枝型(24个)和无血管型(16个)为主;乳腺癌肿54个,血管形态以残根型(19个)和蟹足型(16个)为主,其血管形态分布差异有统计学意义(χ~2=38.78,P<0.01)。Smart 3D SMI诊断乳腺癌的敏感度、特异度、阳性预测值及阴性预测值分别为64.8%、90.0%、85.4%、74.0%。结论Smart 3D SMI可显示乳腺肿物微血管的三维形态学特征,对乳腺癌有较高的诊断价值。
Objective To investigate the value of smart three-dimensional superb microvascular imaging in differentiating benign and malignant breast tumors.Methods From February 2019 to April 2020,106 patients(114 breast masses)were enrolled in the study,who were diagnosed pathologically in our hospital.The patients were examined with smart 3 D SMI and the 3 D morphology of tumor microvessels was observed and compared with the pathological results to evaluate the diagnostic efficacy.Results Among the 114 breast tumors,60 were benign and Smart 3 D SMI showed vascular morphology mainly linear(14),dendritic(24)and avascular(16).The 54 cases of breast cancer showed vascular morphology mainly residual root type(19)and crab foot type(16).The distribution of vascular morphology was statistically different(χ~2=38.78,P<0.01).The sensitivity,specificity,positive predictive value,negative predictive value of smart 3 D SMI were 64.8%,90.0%,85.4%and 74.0%respectively in the diagnosis of breast cancer.Conclusion Smart 3 D SMI has high diagnostic value for breast cancer by displaying the three-dimensional morphological characteristics of breast tumor microvessels.
