小儿轮状病毒感染性腹泻血清CK、CK-MB水平及其与脱水酸中毒的关系 被引量:2

Serum CK and CK-MB Levels in Children With Rotavirus Infectious Diarrhea and Their Relationship With Dehydration Acidosis

摘要 目的探讨分析小儿轮状病毒感染性腹泻血清CK、CK-MB水平及其与脱水酸中毒的关系,掌握轮状病毒性腹泻患儿血清指标变化规律,以指导临床诊治。方法将收治的88例小儿轮状病毒感染性腹泻病例设为观察组,另取同期住院接受治疗的88例非轮状病毒腹泻患儿为对照组,检测两组患儿血清CK、LDH、HBDH、CK-MB和AST水平,将观察组患儿按照脱水程度不同再次分组,分析各组间脱水情况与各指标之间的关系。结果与对照组相比,观察组患儿CK、LDH、HBDH、CKMB和AST水平明显升高,差异有统计学意义(P<0.05)。按照脱水、酸中毒程度不同对观察组进行分组,各组患儿血清CK、AST、CK-MB表达水平差异有统计学意义(P<0.05);随着患儿脱水和酸中毒程度逐渐加重,CK以及CK-MB两项指标水平明显升高(P<0.05)。结论小儿轮状病毒感染腹泻病例伴有不同程度脱水症状,其中肌酸激酶、肌酸激酶MB同工酶随着脱水和酸中毒程度加重而显著升高,这为临床预测患儿心肌损伤提供了依据,可有效预防小儿病毒性心肌炎,具有重要的参考价值。 Objective To investigate and analyze the serum CK and CK-MB levels in children with rotavirus infectious diarrhea and their relationship with dehydration acidosis,and to grasp the changing regularity of serum indicators in children with rotavirus infectious diarrhea so as to guide clinical diagnosis and treatment.Methods 88 cases of rotavirus infectious diarrhea in children admitted were selected for this study,as observation group;and 88 cases of non-rotavirus diarrhea in the same period were selected as control group.Serum levels of CK,LDH,HBDH,CK-MB and AST were detected of the two groups.The children in the observation group were grouped again according to the degree of dehydration,and the relationship between dehydration and each index were analyzed.Results Compared with the control group,the levels of CK,LDH,HBDH,CK-MB and AST in the observation group were significantly higher.Independent sample t test showed that there was statistical significance(P<0.05).The observation group was grouped according to the degree of dehydration and acidosis.There were statistical significance in the levels of serum CK,AST and CK-MB in each group(P<0.05).As the degree of dehydration and acidosis increased,the levels of CK and CK-MB increased significantly(P<0.05).Conclusion Children with rotavirus-infected diarrhea have different degrees of dehydration symptoms.Creatine kinase and creatine kinase MB isoenzymes increase significantly with the degree of dehydration and acidosis.This provides a basis for clinical prediction of myocardial injury in children,and can effectively prevent viral myocarditis in children.It has important reference value.
