摘要
临床上有各种用于测量颈椎功能状态即颈椎活动度的测试仪器。服务于临床的颈椎活动度测量仪器(cervical range of motion-CROM)须具备一定的可靠性和有效性,而不同测量仪器应用于临床的实际效果也需要得到评估。本文将对不同颈椎活动度测量方法的可靠性和有效性及其在临床上应用进展进行综合介绍和评估。通过横向比对(同年代)、纵深比较(同类型)相关领域研究对CROM的效能与临床前景评估,得出以下结论:CROM作为量化评估颈椎功能状态的关键指标,具有一定的临床应用前景。各种CROM测试仪器为临床提供了大量的颈椎运动学与功能状态的数据。各种仪器信效度不一,可靠性也有待进一步研究,如信效度可靠性较好的Cybex EDI-320和CA-6000等以及相对而言可靠性较差的目视测量等。在临床实践中,常把CROM测试仪器应用于颈椎病的诊断、预后、治疗效果评价以及划分颈椎功能损伤程度。临床上应根据不同应用情景选择不同的测试仪器。
There are multiple CROM measurement techniques and instruments aimed at assessing the functional capacity of the cervical vertebra in clinical practice.However,different techniques or instruments’reliability and validity in assessing CROM are still not well evaluated.Meanwhile,the clinical utility of different CROM measurement methods is still indefinite.The purpose of this study is to introduce all available methods for measuring CROM and to evaluate their reliability,validity and their clinical application.Through comparison among Evaluation of the efficacy and clinical prospects of CROM in related field both Horizontally and vertically,it is concluded that CROM is a key indicator of quantitative assessing the functional capacity of the cervical vertebra,which can contribute to clinic practice.Multiple CROM measurement instruments provide much information for cervical vertebra kinematics and functional capacity in clinic.The reliability is different while the validity should be evaluated further,such as superior Cybex EDI-320 and CA-6000 and inferior sight measurement.In clinical practice,CROM devices are commonly used in diagnosis,prognosis,evaluating the effects of treatment in patients with cervical pathology and classify the degree of cervical vertebra functional injury.In clinic practice,different measurements apply to different situations.
