摘要
目的探究单轴锁定钢板系统治疗老年骨质疏松性肱骨近端骨折的策略。方法回顾性分析2015年12月至2017年4月本院收治的26例老年骨质疏松性肱骨近端骨折患者的临床资料。其中Neer分型三部分骨折18例,四部分骨折8例,均采用单轴锁定内固定系统(PHILOS系统)。随访6~12个月,观察患者术后骨折愈合情况和Constant-Murley评分。结果骨折均获得愈合,无塌陷及内固定松动,患者术后6个月Constant-Murley评分优良率达88.5%。其中1例术后早期出现脂肪液化坏死、骨质吸收,最终内固定失败去除内固定后畸形愈合。结论单轴锁定钢板系统治疗老年骨质疏松性肱骨近端骨折可获得较为满意的临床效果,术前综合考虑骨折分型、骨质疏松程度,术中充分利用钢板的设计复位及固定,术后抗骨质疏松并制定个体化的康复计划提高临床疗效。
Objective To explore the strategy of uniaxial locking plate system in the treatment of senile osteoporotic proximal humeral fractures.Methods The clinical data of 26 elderly patients with osteoporotic proximal humeral fractures admitted to our hospital from December 2015 to April 2017 were retrospectively analyzed.Among them,18 cases were three-part fracture and 8 cases were four-part fracture by neer classification.They were all treated by uniaxial locking fixation system(PHILOS system)therapy.Followed for 6 to 12 months to observe the fracture healing and postoperative Constant-Murley score.Results The fractures were healed without collapse and internal fixators loosening.The excellent rate of Constant-Murley score was 88.5%after 6 months.One case showed fat liquefaction necrosis,bone resorption and internal fixation failure.The deformity was healed by removing the internal fixator.Conclusion The uniaxial locking plate system has satisfactory clinical effect in the treatment of senile osteoporotic proximal humeral fractures.To take the comprehensive consideration of fracture classification and osteoporosis before operation,fully utilization the design of the steel plate to reset and fix during operation,postoperative anti-osteoporosis and the development of dividualized rehabilitation programs,in order to improve clinical effect.
