摘要
目的探讨急诊科复合伤患者院内死亡的原因,分析危险因素以制定应对措施。方法选取2018年5月至2019年8月我院急诊收治的148例复合伤患者作为研究对象,调查就诊期间院内死亡情况,采用多因素logistic回归分析确定影响院内死亡的危险因素,并据此拟定针对性护理干预措施。结果调查的148例复合伤患者中,院内死亡42例,病死率为28.38%;经多因素logistic回归分析显示,受伤到就诊时间≥1 h、术中低体温、合并休克、ISS评分≥16分是影响急诊复合伤患者院内死亡的危险因素。结论早期对复合伤患者死亡的高危因素进行筛查,并实施一体化救护管理措施,能降低病死率。
Objective To explore the causes of in-hospital death in patients with multiple injuries in the emergency department,analyze the risk factors to formulate countermeasures.Methods We selected 148 cases of patients with multiple injuries admitted to the emergency department of our hospital from May 2018 to August 2019 as the study objects,investigated the in-hospital deaths during the period of visit,and used multivariate logistic regression analysis to determine the risk factors affecting in-hospital death,and based on this formulated targeted nursing intervention measures.Results Among the 148 cases of patients with multiple injuries investigated,42 cases died in the hospital,with a fatality rate of 28.38%.Multivariate logistic regression analysis showed that the time from injury to treatment≥1 h,intraoperative hypothermia,combined shock,ISS score≥16 points were the risk factors that affected the death in the hospital of patients with multiple injuries in the emergency department.Conclusion Early screening of high-risk factors for death of patients with multiple mortality rate,implementation of integrated rescue management measures,which can reduce the mortality rate.
