摘要
目的分析64层螺旋CT用于胸部创伤诊断中的价值。方法选取2017-12至2019-04本院收治的60例胸部创伤患者进行研究,所有患者于病情确诊前,均开展64层螺旋CT、X线检查诊断,对比两种检查在胸部创伤诊断中的符合率,同时分析胸部创伤CT影像学表现。结果64层螺旋CT共检出58例,X线检查检出胸部创伤者48例。检出率相比,64层螺旋CT明显高于X线(P<0.05),其中,肺部改变检查符合率64层螺旋CT明显高于X线片(P<0.0),但两种检查在支气管撕裂伤诊断中,差异不明显(P>0.05);胸部软骨损伤诊断64层螺旋CT明显高于X线片(P<0.05),但两种检查在胸骨骨折诊断中,差异无统计学意义(P>0.05);且气胸、血胸、血气胸诊断符合率相比,64层螺旋CT、X线片无差异(P>0.05);纵隔血肿与气肿诊断时,64层螺旋CT明显高于X线片(P<0.05),但其在血气肿、纵隔血肿诊断中,差异无统计学意义(P>0.05);膈肌损伤诊断符合率相比,64层螺旋CT、X线片,差异无统计学意义(P>0.05)。结论在胸部创伤诊断中,64层螺旋CT诊断价值较高,可为临床提供直观、精确的诊断依据,值得临床借鉴。
Objective To analyze the value of 64-slice spiral CT in the diagnosis of thoracic trauma.Methods 60 patients with thoracic trauma admitted to our hospital from December 2017 to April 2019 were enrolled in this study.All patients were examined by 64-slice spiral CT and X-ray before the diagnosis of the disease.The coincidence rates of the two examinations in the diagnosis of thoracic trauma were compared,and CT imaging manifestations of thoracic trauma were analyzed.Results A total of 58 cases were detected by 64-slice spiral CT,and 48 cases were detected by X-ray examination.In terms of the detection rate,64-slice spiral CT was significantly higher than X-ray(P<0.05).Among them,the coincidence rate of lung change examination was significantly higher than that of X-ray(P<0.05),but no difference was found in the diagnosis of bronchial laceration between the two examinations(P>0.05);In terms of thoracic cartilage injury,the diagnosis rate of 64-slice spiral CT was significantly higher than the X-ray(P<0.05),but no difference was found in the diagnosis of sternal fracture between the two examinations(P>0.05);In terms of diagnostic coincidence rate of pneumothorax,hemothorax,and hemopneumothorax,there was no statistical difference between 64-slice spiral CT and X-ray(P>0.05);With respect to mediastinal hematoma and emphysema,the diagnosis rate of 64-slice spiral CT was significantly higher than X-ray(P<0.05);With regards to the diagnostic coincidence rate of diaphragm injury,64-slice spiral CT and X-ray films showed no significant difference(P>0.05).Conclusions In the diagnosis of thoracic trauma,64-slice spiral CT is of high diagnostic value,which can provide visualized and accurate diagnosis basis for the clinic and is worthy of clinical reference.
