摘要
目的探讨PICC手臂操对经外周中心静脉置管(PICC)患者置管侧关节功能的影响。方法选取2017年1月至2018年3月南宁市第一人民医院行PICC置管患者87例,采取随机数字表法分为观察组43例和对照组44例。对照组给予PICC置管后常规功能锻炼,观察组给予PICC手臂操功能锻炼。干预4 w后,比较两组患者置管侧肢体腕关节、肘关节、肩关节功能,并观察两组发生置管并发症的情况。结果①两组患者腕关节功能比较差异无统计学意义(P>0.05);观察组肘关节稳定性及日常生活功能优于对照组,差异有统计学意义(P<0.05);观察组肩关节肌力、活动水平、关节活动等均优于对照组,差异有统计学意义(均P<0.05)。②两组患者的静脉血栓、机械性静脉炎、肢体肿胀等并发症发生率比较差异均无统计学意义(均P>0.05)。结论PICC手臂操可维持和促进PICC置管侧肩、肘关节的功能,减少并发症的发生。
Objective To explore the effect of PICC arm exercises on the lateral joint function in patients with peripheral central venous catheterization(PICC).Methods From January 2017 to March 2018,87 patients with PICC catheter in the First People's Hospital of Nanning City were selected and divided into observation group(43 patients)and control group(44 patients)by random number table method.The control group was given routine functional exercise after PICC catheterization,while the observation group was given PICC arm exercise.After 4 weeks of intervention,the functions of wrist,elbow and shoulder joints of the extremities on the side of catheterization were compared between the two groups,and the complications of catheterization were observed.Results①There was no significant difference in wrist function between the two groups(P>0.05).The stability of elbow joint and daily life function in the observation group were better than those in the control group,and the difference was statistically significant(P<0.05).The muscle strength,activity level and joint activity of the observation group were all superior to the control group,and the differences were statistically significant(P<0.05).②There was no statistically significant difference in the incidence of venous thrombosis,mechanical phlebitis,limb swelling and other complications between the two groups(P>0.05).Conclusions PICC arm exercises can maintain and promote the function of the lateral shoulder and elbow joints of PICC catheterization,and reduce the occurrence of complications.
