Application of preoperative multicomponent apheresis collection in aortic dissection operation

摘要 目的分析评价深度自体血采集与回输在主动脉夹层术中应用的意义。方法将青岛大学附属医院2015—2017年主动脉夹层患者分为深度自体血采集组(n=46例)和非深度自体血采集组(n=76例),对两组患者的手术前后各项指标进行统计描述,并通过t检验比较其差异。结果深度自体血采集组与非深度自体血采集组比较:术前血小板与血红蛋白数值无明显差异;术后1d、术后3d、术后7d的血小板与血红蛋白数值差异有统计学意义(P<0.05);两组患者手术出血量无明显差异,而术中红细胞、血浆、血小板、冷沉淀等血液成分输注量差异有统计学意义(P<0.05);其他临床指标比较:住院天数、死亡率和手术血制品花费差异有统计学意义(P<0.05)。结论深度自体血采集有利于降低主动脉夹层患者术中异体用血量,有利于术后恢复,对减少住院天数、降低死亡率具有重要意义。 Objective To analyze and evaluate the significance of multicomponent apheresis collection and retransfusion for patients undergoing aortic dissection operation. Methods Preoperative autologous blood donation(PABD) using whole blood collection(n=46) and PABD using multicomponent(platelet, red cells and plasma) apheresis collection(n=76) in patients undergoing elective aortic dissection(from 2015 to 2017) were compared statistically using t-test in terms of the hemodynamic recovery and clinical outcomes. Results There were no differences in preoperative Plt and Hb levels, or operative blood loss between the two groups(P>0.05), whereas there were significant differences in postoperative Plt and Hb levels at d1,3,7, and intraoperative usage of blood components(including red cells, plasma, platelets and cryoprecipitate etc.)(P<0.05). No significant difference was noted in the length of hospital stay, mortality or blood product costs between the two groups(P<0.05). Conclusion The multicomponent apheresis collection is beneficial to minimize the exposure to allogeneic blood for patients undergoing aortic dissection;it also has positive effects on recovery, hospital stay and mortality of such patients.
