摘要
目的分析夜间配戴角膜塑形镜对不同屈光度青少年近视患者泪液的影响。方法我院收治的青少年近视患者150例共292只眼,按屈光度不同分为低度近视组(-3.00^-1.00 D)65例128只眼,中度近视组(-6.00^-3.25 D)85例164只眼,夜间配戴角膜塑形镜矫正近视,比较不同时间点两组泪膜破裂时间(BUT),泪液基础分泌量SchirmerⅠ试验(SIT),角膜上皮荧光素染色检查(CFS)的变化情况,并观察不良反应。结果与戴镜前比较,戴镜后1周、1月、3月、6月两组BUT显著缩短(P<0.05),戴镜后1周BUT缩短较明显,1月时BUT缩短最显著,戴镜后3月BUT恢复上升,低度近视组BUT大于中度近视组(P<0.05);戴镜后1周、1月、3月、6月两组SIT无显著变化,组间比较差异无统计学意义(P>0.05);戴镜后CFS以Ⅰ级染色为主,戴镜后1周、1月、3月、6月两组CFS例数显著增多(P<0.05),但组间比较差异无统计学意义(P>0.05),戴镜过程中,两组患者眼压均正常,未出现明显不良反应。结论夜间配戴角膜塑形镜矫治不同屈光度青少年近视,对患者泪液基础分泌量无明显影响,但会缩短泪膜破裂时间,增加角膜上皮荧光素染色,建议患者戴镜后按时进行复查。
Objective To analyze the effects of overnight orthokeratology lens on tears in adolescent myopic patients with different diopters.Methods A total of 292 eyes of 150 adolescent myopic patients admitted to our hospital were divided into low myopia group(-3.00^-1.00 D,n=65 and 128 eyes)and moderate myopia group(-6.00^-3.25 D,n=85 and 164 eyes)according to the different diopters.The patients were given orthokeratology lens at night to correct myopia.Before wearing lens,after 1 week,1 month,3 months and 6 months of wearing lens,tear break-up time(BUT),tear basal secretion estimated by using Schirmer I test(SIT)and corneal epithelial fluorescein staining(CFS)were compared between the two groups.The occurrence of adverse reactions was observed.Results Compared with before wearing lens,after 1 week,1 month,3 months and 6 months after wearing lens,the BUT in the two groups was significantly shortened(P<0.05),and the BUT improvement was more significant after 1 week of wearing lens,and the BUT improvement was most significant after 1 month,and the BUT was recovered after 3 months of wearing lens.The BUT in the low myopia group was greater than that in the moderate myopia group(P<0.05).There was no significant change in the SIT between the two groups after 1 week,1 month,3 months and 6 months of wearing lens(P<0.05),and there was no statistically significant difference between the two groups(P>0.05).CFS was mainly grade I staining after wearing lens,and the number of CFS cases was significantly increased after 1 week,1 month,3 months and 6 months of wearing lens when compared with that before wearing lens(P>0.05),and there was no statistically significant difference between the two groups(P>0.05).During wearing lens,the intraocular pressure was normal in the two groups,and there were no obvious adverse reactions.Conclusion Overnight orthokeratology lens to treat adolescent myopia in different diopters has no effects on tear secretion amount,but it could shorten the tear break-up time and increase the staining of corneal epi
