雷火灸联合植骨术干预胫骨干骨折延迟愈合的疗效观察及对血清TGF-β、VEGF的影响 被引量:7

摘要 目的观察雷火灸联合植骨术干预胫骨干骨折延迟愈合的临床疗效及对血清TGF-β、VEGF的影响。方法将70例胫骨干骨折不愈合患者随机分为对照组和观察组,每组35例。对照组予植骨术治疗,观察组予雷火灸联合植骨术治疗。在术后第1、2、3个月予X线骨痂评分,并记录骨折临床愈合时间,测定血清转化生长因子-β(TGF-β)、血管内皮生长因子(VEGF)水平,评价其临床疗效。结果观察组总有效率明显高于对照组(P<0.05);在术后第1、2、3个月,观察组的X线骨痂评分均明显高于对照组(P<0.05);观察组患者的骨折临床愈合时间明显短于对照组(P<0.05)。治疗后,观察组患者血清TGF-β、VEGF水平明显升高(P<0.05),与对照组比较,差异有统计学意义(P<0.05)。结论雷火灸联合植骨术干预胫骨干骨折延迟愈合的疗效确切,可促进骨痂形成,加快骨折愈合,这可能与其改善血清TGF-β、VEGF水平有关。 Objective To observe the clinical efficacy of thunder-fire moxibustion plus bone graft in intervening delayed union of tibial shaft fracture and its effects on serum transforming growth factor-β(TGF-β)and vascular endothelial growth factor(VEGF).Method Seventy patients with delayed union of tibial shaft fracture were randomized into a control group and an observation group,with 35 cases in each group.The control group was given bone graft treatment,while the observation group was treated with thunder-fire moxibustion plus bone graft.Bone callus was scored based on X-ray examination at the 1st,2nd and 3rd months after operation,the clinical fracture healing time was recorded,and the serum levels of TGF-βand VEGF were determined.The clinical efficacy was evaluated.Result The total effective rate was significantly higher in the observation group than in the control group(P<0.05);at the 1st,2nd and 3rd months after operation,the X-ray bone callus scores were significantly higher in the observation group than in the control group(P<0.05);the clinical fracture healing time was markedly shorter in the observation group than in the control group(P<0.05).After treatment,the serum levels of TGF-βand VEGF increased significantly in the observation group(P<0.05),and were significantly different from those in the control group(P<0.05).Conclusion Thunder-fire moxibustion plus bone graft can produce valid efficacy in intervening delayed union of tibial shaft fracture.It can promote the formation of bone callus and the healing of fracture,which may be related to the improvements of the serum levels of TGF-βand VEGF.
