摘要
目的探讨精细化营养护理对食管癌合并2型糖尿病患者血糖控制的影响。方法选择2017年3月—2019年4月在该科收治的食管癌合并2型糖尿病患者65例,随机分为观察组35例及对照组30例。对照组采用常规护理,观察组在常规护理的基础上,给予精细化营养护理护理干预,比较两组患者体重、餐后2 h血糖控制水平、营养指标的差异。结果营养干预后,观察组患者平均体重高于对照组,差异有统计学意义(t=12.845,P<0.001)。观察组患者住院期间完成256次餐后2 h血糖测量,平均血糖水平为(8.8±1.7)mmol/L,对照组住院期间完成223次餐后2 h血糖测量,平均血糖水平(9.3±1.6)mmol/L,差异有统计学意义(t=3.300,P=0.001)。观察组出院时的PG-SGA评分为(6.4±4.2)分,显著低于对照组的(8.5±3.9)分,差异有统计学意义(t=2.077,P=0.042)。结论精细化营养护理可以减少食管癌合并2型糖尿病患者同步放化疗期间的体重下降,提高血糖控制水平,改善患者营养状态,具有重要的临床价值。
Objective To explore the effect of refined nutritional care on blood glucose control in patients with esophageal cancer complicated with type 2 diabetes.Methods From March 2017 to April 2019,65 esophageal cancer patients with type 2 diabetes treated in our department were randomly divided into observation group(35 cases)and control group(30 cases).The control group adopted routine nursing,and the observation group received fine nutritional nursing intervention on the basis of routine nursing,and compared the differences in body weight,blood glucose control level and nutritional indicators at 2 hours after meal between the two groups.Results After nutritional intervention,the average weight of the observation group was higher than that of the control group,the difference was statistically significant(t=12.845,P<0.001).The observation group completed 256 blood glucose measurements 2 hours after a meal during hospitalization,and the average blood glucose level was(8.8±1.7)mmol/L.The control group completed 223 blood glucose measurements 2 hours after a meal during hospitalization,and the average blood glucose level was(9.3±1.6)mmol/L;the difference is statistically significant(t=3.300,P=0.001).The PG-SGA score of the observation group at discharge was(6.4±4.2)points;it was significantly lower than that of the control group(8.5±3.9)points,and the difference was statistically significant(t=2.077,P=0.042).Conclusion Refined nutritional care can reduce the weight loss of patients with esophageal cancer complicated with type 2 diabetes during concurrent radiotherapy and chemotherapy,improve blood glucose control level,and improve the nutritional status of patients,which has important clinical value.
