摘要
目的探索凝血酶原活动度(prothrombin time activity,PTA)与慢性心力衰竭(心衰)患者发生院内死亡的相关性。方法回顾性分析中国医科大学附属盛京医院2013年1月至2018年12月收治的慢性心衰患者3494例,记录患者入院时PTA,按照是否发生院内死亡进行分组,对各组数据进行统计学分析,比较各组差异。运用Spearman相关分析及多元逐步回归分析,分析出与PTA密切相关的因素。利用多因素logistic回归分析得出预测慢性心衰院内死亡的独立危险因素。结果院内死亡组129例,生存组3365例。院内死亡组PTA较生存组明显降低[76%)61%~91%)对88%)75%~99%),P<0.001]。多元逐步回归分析显示PTA与白蛋白、总胆固醇呈正相关;PTA与脑钠肽(BNP)、总胆红素呈负相关;PTA与性别相关(P均<0.001)。多因素logistic回归分析得出PTA降低是慢性心衰患者院内死亡的独立危险因素。结论PTA降低是慢性心衰患者发生院内死亡的独立危险因素。
Objective To investigate the correlation between PTA and in-hospital mortality in patients with chronic heart failure.Methods A total of 3494 chronic heart failure patients who had prothrombin time activity(PTA)levels measured on admission were in Shengjing Hospital between Jan.2013 and Dec.2018.Subjects were divided into groups according to whether there was in-hospital mortality or not.The data of each group were statistically analyzed and the differences of each group were compared.The factors strictly related to PTA were discovered by Spearman’s rank correlation analysis and multivariate stepwise regression analysis.Multiple factor logistic regression analysis were used to obtain independent risk factors for predicting in-hospital mortality in chronic heart failure.Results There were 129 in-hospital death group and3365 in survival group in this study.PTA in the in-hospital death group was significantly lower than that in the survival group[76%(61%~91%)vs.88%(75%~99%),P<0.001].Multivariate stepwise regression analysis revealed that PTA was positively correlated with albumin and total cholesterol,negatively correlated with BNP and total bilirubin,PTA was correlated with gender(P<0.001).Multiple factor logistic regression analysis revealed that decreased PTA was the independent risk factors of in-hospital mortality in patients with chronic heart failure.Conclusion Decreased PTA is an independent risk factor of in-hospital mortality in patients with chronic heart failure.
