摘要
目的评价2008-2018年在云南省首次接受免费抗病毒治疗艾滋病病毒(HIV)阳性女性商业性行为者的治疗效果并探讨影响因素。方法选择2008-2018年间在云南省首次接受抗病毒治疗的311例HIV阳性女性商业性行为者为研究对象,回顾性分析治疗前后CD4^+T淋巴细胞(简称CD4细胞)计数变化和治疗6个月后的病毒载量情况,对可能促进CD4细胞数上升的因素及影响抗病毒治疗有效率的因素分别进行Logistic回归分析。结果311例HIV阳性女性商业性行为者292例治疗时间>6个月,治疗有效率为91.10%(266例)。Logistic分析显示,基线CD4细胞为350~499个/μL和基线CD4细胞≥500个/μL相对于基线<200个/μL是促进CD4细胞数上升保护因素;治疗时间为13~24个月、25~36个月和≥49个月的相对于0~12个月是促进CD4细胞数上升的危险因素;年龄为20~29岁、30~39岁、40~49岁、≥50岁相对于15~19岁是抗病毒治疗有效率危险因素。结论云南省接受抗病毒治疗HIV阳性女性商业性行为者治疗效果较好,可继续推行HIV阳性女性商业性行为者"发现即治疗""尽早治疗"抗病毒治疗策略,同时做好依从性教育。
Objective To evaluate the therapeutic effect of free antiviral therapy(ART)for HIV-positive female sex workers(FSWs)during 2008-2018 and to explore the influencing factors in Yunnan province.Methods A total of 311 HIV-positive FSWs receiving ART for the first time from 2008 to 2018 were selected.The changes in CD4 counts before and after ART and the viral load after 6 months of ART were retrospectively analyzed.Logistic regression analysis was used to analyze the factors which might increase CD4 cells and influence the efficacy of ART.Results Out of the 311 cases,292 cases were treated for more than 6 months,the effective rate of ART was 91.09%.Logistic analysis showed that the CD4 cells of 350-499 and≥500 at baseline were protective factors compared with the baseline<200;treatment durations of 13-24 months,25-36 months,and≥49 months were risk factors compared with that of 0-12 months.The age groups of 20-29,30-39,40-49,≥50 years were the risk factors for the effective rate of ART compared with the group of 15-19 years.Conclusion The treatment effect of HIV-positive female sex workers is satisfactory in Yunnan.The strategy of"treatment right after HIV diagnosis"should be continued to implement,and at the same time attention be paid to adherence education.
