摘要
目的研究脐带搏动停止后实施断脐操作对母婴结局的影响。方法选取2016年4月至2017年4月在本院进行阴道分娩的5000例产妇为研究对象,根据随机数字表法将其分为对照组和观察组,各2500例。对照组进行早断脐操作,观察组于脐带搏动停止后实施断脐操作。比较两组产妇分娩情况及新生儿健康状况。结果观察组产后24 h出血量显著少于对照组(P<0.05)。观察组新生儿产后第1、7天的Hb及HCT水平均显著高于对照组(P<0.05);两组新生儿产后第7天的Hb及HCT均高于产后第1天(P<0.05)。两组新生儿产后第1、7天的TCB及STB水平比较,差异无统计学意义(P>0.05);两组新生儿产后第7天的TCB及STB水平均高于产后第1天(P<0.05)。观察组新生儿的窒息、贫血发生率均明显低于对照组(P<0.05)。结论在脐带搏动停止后实施断脐操作能够减少产妇产后出血量,改善新生儿结局,有助于保证母婴安全。
Objective To study the effect of umbilical cord transection after umbilical cord pulsation cessation on maternal and infant outcomes.Methods A total of 5000 pregnant women who had vaginal delivery in our hospital from April 2016 to April 2017 were selected as the research objects.According to the random number table method,the pregnant women were divided into control group and observation group,with 2500 cases in each group.The control group was given early umbilical cord transection,and the observation group was given umbilical cord transection after umbilical cord pulsation cessation.The delivery of pregnant women and health status of neonates of the two groups were compared.Results The 24 h postpartum blood loss volume in the observation group was significantly less than that in the control group(P<0.05).The levels of Hb and HCT of neonates on the first and seventh days after delivery in the observation group were significantly higher than those in the control group(P<0.05);the levels of Hb and HCT of neonates on the seventh days after delivery were higher than those on the first day after delivery in both groups(P<0.05).There were no significant differences in the levels of TCB and STB of neonates on the first and seventh days after delivery between the two groups(P>0.05);the levels of TCB and STB of neonates on the seventh days after delivery were higher than those on the first day after delivery in both groups(P<0.05).The incidences of asphyxia and anemia of neonates in the observation group were significantly lower than those in the control group(P<0.05).Conclusion Umbilical cord transection after umbilical cord pulsation cessation can decrease the postpartum blood loss volume,improve neonate outcomes,help to ensure the safety of mother and infant.
