摘要
目的探讨断指再植术后两点辨别觉差异性恢复的原因并提出康复护理策略。方法选取2015年6月-2017年12月我院成功行断指再植术的125例患者,根据两点辨别觉(Two-point discrimination,2PD)恢复情况分为2PD<6 mm组81例,2PD 6~8 mm组32例,2PD 9~10 mm组12例,对影响断指再植术后两点辨别觉的相关因素进行单因素分析,选取差异有统计学意义的指标,进行多因素Logistic回归分析,并提出康复护理策略。结果单因素分析及多因素Logisic回归分析显示:康复训练、损伤机制是断指再植术后两点辨别觉差异性恢复的相关影响因素。年龄、保存方式、职业在单因素分析中有统计学意义(P<0.05),而在多因素Logisic回归分析中无意义。结论早期识别断指再植术后两点辨别觉恢复的影响因素,采取有效防范措施,术后通过感觉的康复训练,促进患指感觉功能早期恢复,在临床工作能够采取更为确切的评判标准来推测患者感觉功能的恢复。
Objective To analyze the causes of the differential recovery of the two-point discrimination after replantation of severed fingers,and to propose the rehabilitation strategy to provides clinical date and promotes the early recovery of the affected finger's sensory function.Methods From June 2015 to December 2017,125 patients who successful finger replantation in our hospital were selected.According to the two-point discrimination recovery,81 cases were divided into the 2PD<6 mm group,32 cases were in the 2PD 6~8 mm group,and 12 cases were in the 2PD 9~10 mm group.Its influence factors was measured by univariate analysis and Logisic regression analysis.Results The influence factors include rehabilitation training,damage mechanism,ages,method of preservation.Occupation were statistically significant in univariate analysis and non significant in multivariate logistic regression analysis.Conclusions To identify the factors that influence the recovery of two-point discrimination after replantation of severed finger earlier,effective preventive measures should be taken to promote early recovery of the affected finger's sensory function through sensory rehabilitation training and more accurate evaluation criteria can be adopted in clinical work to predict it.
