摘要
目的观察传统睾丸下降固定术与腹腔镜手术在小儿高位隐睾中的应用效果。方法98例高位隐睾患儿分为对照组与观察组,分别采用传统睾丸下降固定术治疗和腹腔镜手术治疗。比较2组的手术治疗效果、手术时间、术中出血量、治疗后睾丸容积及免疫功能情况。结果观察组治疗优良率(95.92%)与对照组(91.84%)差异无统计学意义(P>0.05),但手术时间、术后住院时间、术中出血量均少于对照组,差异有统计学意义(P<0.05)。术后6个月,2组睾丸容积均高于同年龄段治疗前,差异有统计学意义(P<0.05),但组间差异无统计学意义(P>0.05)。与术前1 d比较,2组术后1 d的TNF-α、IL-6含量升高,CD3、CD4细胞比例下降,差异有统计学意义(P<0.05)。与术后1 d比较,2组术后4 d的TNF-α、IL-6含量下降,CD3、CD4细胞比例升高,差异有统计学意义(P<0.05)。结论腹腔镜手术治疗小儿高位隐睾,临床效果与传统睾丸下降固定术相似,但手术时间、住院时间更短,术后急性免疫抑制作用弱。
Objective To observe the effect of traditional cryptorchidopexy and pediatric laparoscopic surgery in high cryptorchidism.Methods Ninety-eight children with high cryptorchidism admitted to Yongkang Maternal and Child Health Hospital from January 2016 to December 2018 were randomly divided into the control group and the observation group.The control group was treated with traditional cryptorchidopexy and the observation group was treated with laparoscopic surgery.The effect of the surgery,operation time,intraoperative blood loss,and other surgical parameters were compared between the two groups,as well as the change of testicular volume and immune function after treatment.Results The total rate of excellent and good of the observation group was 95.92%,whereas the rate of the control group was 91.84%.The difference was not statistically significant(P>0.05).The operation time and postoperative hospital stay in the observation group were shorter than those in the control group.The intraoperative blood loss was less than that in the control group(P<0.05).There was no significant difference in postoperative exhaust time between the two groups(P>0.05).The testicular volume of the two groups was higher than that of the same age group(P<0.05).There was no significant difference in testicular volume between the observation group and the control group at the same age group before and after 6 months(P>0.05).The levels of TNF-αand IL-6 in the two groups were significantly higher than those on the first day after the operation.The proportion of CD3 and CD4 cells was significantly lower than that before surgery.The levels of TNF-αand IL-6 in the two groups were significantly lower than those at 1 day after the operation,and the proportion of CD3 and CD4 cells was significantly higher than that at 1 day after the operation(P<0.05).Conclusions Pediatric laparoscopic surgery for high cryptorchidism has a significant clinical effect,short operation time,and hospital stay,and postoperative acute immunosuppression is mild.
