摘要
目的:探究磁共振成像(MRI)联合乳腺影像报告及数据系统(BI-RADS)分级在老年乳腺癌患者临床诊断中的应用价值。方法:选取274例老年乳腺癌患者为研究对象,共收集290个病灶。对所有病灶进行临床病理检测区分良恶性;使用MRI联合BI-RADS对病灶形状、边缘、内部强化、早期强化、TIC类型等病灶征象进行检测;结合ROC分析法,评估MRI联合BI-RADS征象检验对不同类型病变样本的灵敏度、特异度和准确度。结果:病理检测发现,290个病灶中良性病灶104个,恶性病灶182个;肿块型病变253个,非肿块型病变37个。病灶征象检测比较显示,肿块型病变和非肿块型病变中病灶形状、边缘、内部强化、早期强化、TIC类型数据比较有统计学意义(P<0.01)。MRI联合BI-RADS检验方法的诊断价值高(P<0.05),对肿块型病变的诊断灵敏度、特异度及准确度分别为0.904、0.880和0.893。对非肿块型病变的诊断灵敏度、特异度及准确度分别为0.880、0.833和0.891。结论:MRI联合BI-RADS分级在老年乳腺癌患者的临床诊断中有很高的应用价值,尤其是对于肿块型病变的诊断能够达到较高的预测水平,临床上值得推广应用。
Objective:To explore the application value of magnetic resonance imaging(MRI)combined breast imaging reporting and data system(BI-RADS)classification in the clinical diagnosis of elderly breast cancer patients.Methods:274 elderly breast cancer patients were selected for the study,and a total of 290 lesions were collected.Clinical pathological testing of all lesions was used to distinguish benign and malignant,using MRI combined BI-RADS to test and contrast the lesions shape,edge,internal reinforcement,early reinforcement,TIC type and other lesions.Combined with ROC analysis,the sensitivity,specificity and accuracy of MRI combined with BI-RADS symptom test for different types of lesions were evaluated.Results:Among the 290 lesions,104 were benign,182 were malignant,253 were mass lesions and 37 were non-mass lesions.The comparison of the detection of lesion signs showed that the comparison of the lesion shape,edge,internal enhancement,early enhancement and TIC type data in the mass lesions and non-mass lesions had significant statistical significance(P<0.01).The diagnostic value of MRI combined with the BI-RADS test was high(P<0.05),and the diagnostic sensitivity,specificity and accuracy of the mass lesions were 0.904,0.880 and 0.893,respectively.The diagnostic sensitivity,specificity and accuracy of the non-mass lesions were 0.880,0.833 and 0.891.Conclusion:MRI combined with BI-RADS classification has a high application value in the clinical diagnosis of elderly breast cancer patients,especially for the diagnosis of mass lesions can reach a high level of prediction,clinical application can be widely promoted.
