摘要
目的:探讨控制性超促排卵黄体期患者血清孕酮(P)、雌二醇(E 2)及白细胞介素-1β(IL-1β)水平对胚胎着床的影响。方法:选择80例体外受精-胚胎移植(IVF-ET)患者,所有患者均采用黄体期长方案促排卵治疗,于新鲜周期移植后第7天采集静脉血检测血清P、E 2及IL-1β水平,观察胚胎着床妊娠情况,对比妊娠组(n=31)与非妊娠组(n=49)血清指标的差异,通过Logistic回归分析血清P、E 2及IL-1β水平对胚胎着床的影响。结果:妊娠组获卵数、受精率、优质胚胎率均高于非妊娠组(P<0.05);妊娠组血清P、IL-1β水平以及P/E 2比值高于非妊娠组(P<0.05);妊娠组E 2水平低于非妊娠组(P<0.05);黄体期血清P/E 2值为300~400时的妊娠率62.50%,P/E 2值<300时的妊娠率27.27%,P/E 2值≥400时的妊娠率33.33%(P<0.05);黄体期血清IL-1β≥80 pg/ml时妊娠率达到64.52%,高于IL-1β<80 pg/ml的妊娠率22.45%(P<0.05);Logisitic回归分析显示年龄、移植胚胎级别、血清P/E 2、IL-1β是着床成功的影响因素。结论:黄体期长方案超促排卵后黄体期血清P/E 2比值为300~400,IL-1β水平高于80 pg/ml时着床成功的可能性更高。
Objective:To explore the effects of serum progesterone(P),estradiol(E 2)and interleukin-1β(IL-1β)levels on embryo implantation after superovulation during progesational stage.Methods:Eighty patients who underwent in vitro fertilization-embryo transfer(IVF-ET)were enrolled.All were treated with ovulation induction.On the 7th day after egg retrieval,venous blood was collected to detect levels of serum P,E 2 and IL-1β.The embryo implantation and pregnancy were observed.The differences in serum indexes were compared between pregnancy group(n=31)and non-pregnancy group(n=49).The effects of serum P,E 2 and IL-1βlevels on embryo implantation were analyzed by Logistic regression analysis.Results:The number of retrieved oocytes,fertilization rate and high-quality embryos rate in pregnancy group were higher than those in non-pregnancy group(P<0.05).The levels of serum P and IL-1β,and P/E 2 in pregnancy group were higher than those in non-pregnancy group(P<0.05),while E 2 level was lower than that in non-pregnancy group(P<0.05).During progesational stage,the pregnancy rate was 62.50%when P/E 2 was 300-400,27.27%when P/E 2 was less than 300,33.33%when P/E 2 was more than 400(P<0.05);the pregnancy rate in patients with serum IL-1βnot lower than 80pg/ml was higher than that with serum IL-1βlower than 80 pg/ml(64.52%vs.22.45%)(P<0.05).Logisitic regression analysis showed that age,grade of transferred embryos,serum P/E 2 and IL-1βwere influencing factors of successful implantation.Conclusion:When serum P/E 2 is 300-400 and IL-1βlevel is higher than 80 pg/ml after superovulation during progesational stage,the possibility of successful implantation is higher.
