摘要
目的:探讨失眠症患者和伴有失眠的抑郁症患者对安眠药的内隐态度,并分析内隐态度与失眠严重程度、不良情绪以及服药依从性的关系。方法:选取符合国际疾病分类第十版(ICD-10)中相应诊断标准的非器质性失眠症患者(失眠组)63例和伴有失眠症状的抑郁症患者(抑郁组)51例,用E-prime 2.0编制的单类内隐联想测验(SC-IAT)、匹兹堡睡眠质量指数量表(PSQI)、用药态度量表(DAI)和Morisky药物依从性量表(MMAS-8)进行测量,并用汉密尔顿抑郁量表(HAMD)和汉密尔顿焦虑量表(HAMA)评定抑郁、焦虑症状。结果:失眠组内隐D值(0.33±0.39)和抑郁组D值(0.35±0.42)均显著大于0(t=6.384,5.831;P<0.001),两组间内隐效应差异无统计学意义;D值与PSQI总分、HAMD、HAMA、MMAS-8量表得分相关显著(r=0.453,0.298,0.201,-0.320;P<0.05),与DAI得分相关不显著(r=0.011,P=0.908)。结论:失眠症患者和伴有失眠症状的抑郁症患者对安眠药都存在消极的内隐态度,外显态度与内隐态度相对独立,两者是不同的建构。安眠药内隐态度与失眠严重程度、不良情绪和服药依从性均存在不同程度的相关关系。
Objective:To explore the implicit attitude of insomnia patients and depression patients with insomnia to hypnotics,and explore the relationship between implicit attitude and the severity of insomnia,negative emotions and medication adherence.Methods:A total of 63 patients with non organic insomnia and 51 depression patients with insomnia were included in the study,who were conformed to the corresponding diagnostic criteria in International Statistical Classification of Diseases and Related Health Problems 10 th Revision(ICD-10).The SC-IAT was developed with software E-prime 2.0,and the explicit attitude of insomniacs was measured with drug attitude inventory.The Pittsburgh Sleep Quality Index(PSQI)and Eight Item Medication Adherence Scale(MMAS-8)were used for allsubjects.The Hamilton Depression Scale(HAMD)and Hamilton Anxiety Scale(HAMA)were used to assess the severity of depression and anxiety.Results:The mean value of D in insomnia group(0.33±0.39)and depression group(0.35±0.42)were significantly higher than 0(t=6.384,5.831;P<0.001).There was no significant difference between two groups.The mean value of D was significantly correlated with PSQI,HAMD,HAMA and MMAS-8(r=0.453,0.298,0.201,-0.320;P<0.05),but not significantly correlated with DAI score.Conclusion:The implicit attitude of non organic insomnia patients and depression patients with insomnia to hypnotics is negative.The explicit attitude and implicit attitude are independent of each other.The implicit attitude of hypnotics is related to the severity of insomnia,negative emotions and medication adherence.
