摘要
目的探讨肾衰方对慢性肾衰竭(chronic renal failure,CRF)患者肾纤维化程度、氧化应激及相关因子水平的影响。方法选取2018年2月—2019年3月收治的慢性肾衰竭患者74例,依据随机数字表法随机分为对照组和治疗组,各37例,对照组给予常规西医治疗,治疗组在对照组的基础上应用肾衰方进行治疗,两组患者进行为期8周的治疗。观察两组患者中医症状积分改变情况,治疗前后患者肾功能、肾纤维化及氧化应激相关指标改善情况,并比较两组临床疗效。结果对比两组患者治疗前后中医症状积分变化,治疗组患者症状积分降低较对照组更为明显,结果具有统计学意义(P<0.05);两组治疗后肾功能相关指标肌酐(Scr)、尿素氮(Bun)及肾纤维化指标血清胱抑素(Cys C)、血清透明质酸(HA)均明显改善(P<0.05),且治疗组较对照组改善更显著(P<0.05);两组患者治疗后氧化应激指标超氧化物歧化酶(SOD)明显升高(P<0.05),丙二醛(MDA)呈下降趋势(P<0.05),且治疗组变化程度更为明显,说明治疗组氧化应激功能改善显著(P<0.05);治疗后,治疗组临床总有效率为91.89%,对照组临床总有效率为75.68%,治疗组的临床疗效优于对照组(P<0.05)。结论肾衰方具有补脾益肾、利水活血、解毒降浊之效,可以改善慢性肾衰竭患者临床症状和体征,减轻肾纤维化程度,改善肾功能,减轻氧化应激损伤,对于慢性肾衰竭有较好的的治疗效果,值得临床推广应用。
Objective The purpose of this study was to investigate the effect of Shenshuai Decoction on renal fibrosis,oxidative stress and related factors in patients with chronic renal failure. Methods 74 chronic renal failure patients admitted to our hospital from February 2018 to March 2019 were selected and randomly divided into treatment group and control group with 37 cases in each group according to the table of random number. The control group was administered with conventional western treatment,while the treatment group was administered with Shenshuai Decoction on the basis of the control group. The patients in both groups were treated for 8 weeks. Observed the changes of TCM symptom scores,improvement of renal function,renal fibrosis and indexes related to oxidative stress before and after treatment in the two groups,and compared the clinical efficacy between the two groups. Results Comparing the changes of TCM symptom scores before and after treatment between the two groups,the reduction of symptom scores in the treatment group was more significant than that in the control group,with significant difference(P<0.05). After treatment,the renal function related indexes including Scr and Bun,and renal fibrosis indexes including serum Cys C and HA were significantly improved in the two groups(P<0.05),and the improvements in the treatment group were more significant than that in the control group(P<0.05). After treatment,the superoxide dismutase(SOD),which is oxidative stress index,increased significantly(P<0.05)and malondialdehyde(MDA)decreased(P<0.05)in both groups,and the change degree in the treatment group was more obvious,indicating that the oxidative stress function in the treatment group improved significantly(P<0.05). After treatment,the total clinical effective rate of the treatment group and control group was 91.89% and 75.68%,respectively.
