摘要
目的:分析某院实施DRG收付费改革对其住院费用及费用结构的影响。方法:收集2015—2018年样本医院的全部出院病人的住院费用,通过指数平滑法对DRG实施后一年的次均住院费用进行预测并与实际费用进行比较分析。采用关键知情人访谈、半结构式访谈等方式对定量分析结果进行补充分析。结果:样本医院的平均住院天数环比增长率下降,诊疗人次、次均住院费用均有所增长,且收治患者的疾病严重程度大幅提升,住院费用结构向合理方向转变。结论:DRG改革初见成效,应持续加强监管,同步配套政策落实完善。
Objective:To analyze the impact of DRG payment reform on the expenses and structure of hospitalization in a hospital.Methods:To collect the hospitalization expenses of all discharged patients of the hospital in this case from 2015 to 2018,and to predict the average hospitalization expenses for one year after the implementation of DRG by the exponential smoothing method and compare the predicted results with the actual expenses.The supplementary analysis of quantitative analysis results is conducted by means of interviews with key insiders and semi-structured interviews.Results:The chain growth rate of the average length of stay in the hospital decreases,the number of inpatients and the average hospitalization expenses increase,and the disease severity of inpatients increases significantly,so the structure of the hospitalization fee changes to a reasonable direction.Conclusion:The DRG reform has achieved initial results.Supervision should be continuously strengthened and the supporting policies should be implemented simultaneously.
