外科给药护理中断事件现状及患者心理行为反应的研究 被引量:7

摘要 目的:调查外科给药过程中护理中断事件的发生现状和患者面对中断事件的心理行为反应方式。方法:采用便利抽样法,以首都医科大学附属北京天坛医院2019年5月6个工作日上午、中午、下午(8∶30—10∶30、11∶30—13∶30、14∶30—16∶30)3个时间段外科6个病区护理班护士在给药过程中(包括药物配置和执行给药2个环节)发生的NI为观察对象。共观察到所有护理班护士36名。记录其开始时间、结束时间、当前事务、来源、类型、结局、患者的心理反应以及患者的行为反应。结果:外科护士在给药的216 h中共发生了1040次护理中断事件,发生频率为4.81次/h,持续时间为(2.34±0.97)min;主要来源为环境,占30.77%(320/1040);类型以侵扰型为主,占69.23%(720/1040);结局消极型为主,占96.9%(1008/1040)。患者对护理中断事件的不满率为43.46%(113/260),77.88%(88/113)的患者不会采取任何行为来阻止中断的继续发生或向护士表达不满。结论:外科给药过程护理中断事件发生频率高,来源多,原因复杂,结局多为消极,且患者有着高不满率和低行为应对方式,因此有必要优化护理工作流程,采取相应的管理措施,避免或减少不必要的中断,保障临床护理安全用药,提高患者满意度。 Objective:To investigate the current status of nursing interruptions during surgical drug administration and the patient's psychological and behavioral response to interruptions.Methods:The nursing interruptions occurring in the nurses from 6 surgical wards during 3 periods(8∶30-10∶30,11∶30-13∶30 and 14∶40-16∶30)during 6 working days in May at Beijing Tiantan Hospital,Capital Medical University were selected by convenient sampling.Totally 36 nurses were observed.The start time,end time,current affairs,source,type,outcome of nursing interruptions as well as the patient's psychological and behavioral response were recorded.Results:Totally 1040 nursing interruptions occurred in surgical nurses within 216 hours of drug administration,with a frequency of 4.81 times/h and a duration of(2.34±0.97)min.The main source of nursing interruptions was the environment,accounting for 30.77%(320/1040).The type of nursing interruptions was mainly intrusive,accounting for 69.23%(720/1040).The outcome was mainly negative,accounting for 96.9%(1008/1040).The dissatisfaction rate of patients with nursing interruptions was 43.46%(113/260),and 77.88%(88/113)patients did not take any action to prevent the interruption or express their dissatisfaction with the nurse.Conclusions:Nursing interruptions in the surgical drug administration process feature a high frequency,multiple sources,and complicated causes.The outcomes are mostly negative,and patients have high dissatisfaction rates and low behavioral coping styles.Therefore,it is necessary to optimize the nursing workflow and take corresponding management measures to avoid or reduce unnecessary interruptions,thereby ensuring the safe use of drugs in clinical care and improving patient satisfaction.
