摘要
目的探讨疏肝活血化瘀方对急性心肌梗死(AMI)经皮冠状动脉介入(PCI)术后患者血清妊娠相关蛋白A(PAPPA)、白介素6(IL-6)和细胞间黏附因子-1(ICAM-1)的影响,为AMI患者PCI手术患者的临床诊治提供参考。方法选取住院行PCI治疗的AMI患者60例,随机分为对照组与观察组,每组30例,术后常规给予抗血小板聚集、β受体阻滞剂、抗心肌缺血等药物治疗,观察组在以上用药基础上给予疏肝活血化瘀方,连续用药7 d,观察患者治疗前后中医证候积分、左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、射血分数(LVEF)、血清PAPPA、IL-6、ICAM-1水平变化。结果两组患者治疗后中医证候主症及次症积分、总积分均明显较治疗前下降(均P<0.001),观察组治疗后主症及次症积分、总积分低于对照组(均P<0.001);两组患者治疗后LVEDD、LVESD较治疗前下降(均P<0.001),LVEF较治疗前升高(均P<0.001),观察组治疗后LVEDD、LVESD低于对照组(P<0.001)、LEVF高于对照组(P<0.001);两组患者治疗后血清PAPPA、IL-6、ICAM-1较治疗前下降(均P<0.001),观察组治疗后以上指标水平低于对照组(均P<0.001);观察组治疗总有效率为93.33%(28/30)高于对照组73.33%(22/30)(P<0.05)。结论AMI行PCI治疗的患者在术后给与疏肝活血化瘀方治疗有助于患者症状及心功能的改善,降低炎症反应,提高治疗疗效。
Objective To investigate the effect of Shugan Huoxue Huayu Prescription on serum PAPPA,IL-6 and ICAM-1 in patients with acute myocardial infarction(AMI)after percutaneous coronary intervention(PCI)to provide reference for clinical diagnosis and treatment of AMI patients undergoing PCI.Methods Sixty cases of patients with acute myocardial infarction after PCI were randomly divided into control group and observation group,30 cases in each group.The postoperative routine administration of anti-platelet aggregation,receptor blockers,anti-myocardial ischemia and other drugs were used.The observation group on the basis of the above drugs was given Shugan Huoxue Huayu Prescription,and continuous medication lasted for 7 days.We observed the TCM syndrome integral,left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD),ejection fraction(LVEF)and serum levels of PAPPA,IL-6 and ICAM-1 before and after treatment.Results The scores of main symptoms,secondary symptoms and total scores of TCM syndromes in both groups after treatment were significantly lower than those before treatment(P<0.001).The scores of main symptoms,secondary symptoms and total scores of TCM syndromes in the observation group were lower than those in the control group after treatment(P<0.001).The LVEDD and LVESD in both groups after treatment were significantly lower than those before treatment(P<0.001).LVEF levels in both groups after treatment were significantly higher than those before treatment(P<0.001).Levels of LVEDD and LVESD in the observation group were lower than those in the control group after treatment(P<0.001).The level of LVEF in the observation group was higher than that in the control group after treatment(P<0.001).The levels of PAPPA,IL-6 and ICAM-1 in both groups after treatment were significantly lower than those before treatment(P<0.001).Levels of PAPPA,IL-6 and ICAM-1 in the observation group were lower than those in the control group after treatment(P<0.001).The total effective rate was 93.33%(28/3
