摘要
目的探讨MR在膝关节外伤性骨挫伤诊断中的准确度及临床意义。方法方便选取2016年10月—2019年2月在该院就诊的87例膝关节外伤性骨挫伤患者作为研究对象,所有患者均行膝关节X线正侧位片及膝关节MR检查,排除股骨、胫骨和髌骨骨折患者,使用SIEMENS 1.5T MR,选择膝关节专用线圈进行扫描。所有患者行斜矢状面(T1WI+质子密度加权成像)、冠状面(质子密度加权成像)、横断面(T1WI+T2WI+质子密度加权成像),设置扫描层厚3.0 mm,层间距0.5 mm。该院高年资影像医师进行阅片,分析患者膝关节MR影像特点。结果股骨外侧髁挫伤最为多见,为28例(占32.2%),其次为胫骨平台挫伤21例(占24.1%),髌骨挫伤16例(占18.4%),股骨内侧髁13例(占14.9%),股骨髁间挫伤9例(占10.3%),骨挫伤伴有不同程度的关节积液、半月板撕裂、内侧副韧带损伤、前交叉韧带损伤、关节软骨损伤、外侧副韧带损伤。结论MR可早期准确诊断膝关节外伤性骨挫伤,避免出现漏诊和误诊情况,而且可以评估损伤部位、范围及是否合并关节积液、半月板或韧带损伤,为临床提供治疗依据,帮助制定治疗计划提高患者预后,控制病情发展及并发症的发生。
Objective To observe the accuracy and clinical significance of MR in the diagnosis of traumatic bone contusion of the knee joint.Methods 87 patients with traumatic bone contusion who were treated in the hospital from October 2016 to February 2019 were convenient selected as the research object.All patients underwent knee joint X-ray and knee MR examination to exclude femur,for patients with fractures of the tibia and patella,use SIEMENS 1.5T MR and select a special coil for the knee joint for scanning.All patients underwent oblique sagittal plane(T1WI+proton density weighted imaging),coronal plane(proton density weighted imaging),and cross-section(T1WI+T2WI+proton density weighted imaging).The scan layer thickness was 3.0 mm and the layer spacing was 0.5 mm.The senior radiologists in our hospital reviewed the images and analyzed the characteristics of the patients'knee MR images.Results The lateral femoral condyle contusion was the most common,with 28 cases(32.2%),followed by tibial plateau contusion,21 cases(24.1%),patella contusion 16 cases(18.4%),and medial femoral condyle 13 cases(14.9%),9 cases of femoral condyle contusion(10.3%),bone contusion with varying degrees of joint effusion,meniscal tear,medial collateral ligament injury,anterior cruciate ligament injury,articular cartilage injury,lateral collateral ligament injury.Conclusion MR can accurately diagnose early traumatic bone contusion of the knee joint to avoid missed diagnosis and misdiagnosis,and can assess the location and extent of the injury and whether it is associated with joint effusion,meniscus or ligament injury,provide clinical treatment basis,help to develop treatment plans to improve the patient's prognosis,controlling the development of the disease and the occurrence of complications.
