摘要
目的探索深圳某医院药品不良反应(ADR)患者的用药特征,为保障患者安全用药提供参考。方法运用潜在类别分析模型对不良反应人群进行异质性识别。结果594名ADR患者中,静脉滴注是引起ADR的主要途径(占65.19%),引起ADR频次最高的药物为抗感染药(占44.12%),ADR报告中以皮肤及其附件损害最多(占36.2%);潜在类别模型将ADR人群分为皮肤损害的ADR患者(占38.89%)、全身损害的ADR患者(占31.48%)和消化道损害的ADR患者(占29.63%)3类,第1类患者主要使用了经静脉注射的抗感染类药物和心血管系统类药物,第2类患者主要使用了经静脉注射的抗感染类药物和体激素类药物,第3类患者主要使用了经其他方式给药的泌尿生殖系统及性激素类药物。结论目前导致不良反应的因素众多,使用潜在类别分析模型能够有效识别出异质亚群体,有助于卫生相关部门根据其人群特征施加个性化的防治策略,从而达到安全用药、降低ADR发生率的目的。
Objective To explore the characteristics of adverse drug reactions(ADR)in a hospital in Shenzhen,and to provide reference for the safety of drug use.Methods The latent class analysis model was used to identify the heterogeneity of the patients with ADR.Results Among the 594 patients with ADR,intravenous drip was the main path to cause ADR,accounting for 65.19%.The drugs with the highest frequency of ADR were anti-infective drugs,accounting for 44.12%.In the ADR report,skin and its accessories were the most damaged,accounting for 36.2%.The latent class analysis model was used to divide ADR population into"patients with skin lesions"(38.89%),"patients with systemic lesions"(31.48%)and"patients with digestive tract lesions"(29.63%).Patients in class 1 were mainly treated with intravenous anti-infective drugs and cardiovascular system drugs.Patients in class 2 were mainly treated with intravenous anti-infective drugs and systemic hormonal preparations.Patients in class 3 were mainly treated with urogenital system and sex hormones drugsadministered by other means.Conclusion At present,there are many factors leading to adverse reactions.The use of latent class analysis can effectively identify heterogeneous subgroups,which is helpful for health related departments to apply personalized prevention and control strategies according to their population characteristics,so as to achieve the purpose of safe drug use and reduce the incidence of ADR.
