摘要
目的探究高龄瘢痕子宫再次妊娠合并重度子痫前期对妊娠结局的影响。方法选择于我院分娩的高龄瘢痕子宫再次妊娠合并重度子痫前期的65例孕产妇作为研究组,另选择同期于我院分娩的单纯高龄瘢痕子宫再次妊娠的73例孕产妇作为对照组。比较两组的凝血功能指标、并发症发生情况、剖宫产率、终止妊娠孕周及妊娠结局。结果入院时,研究组的R、PT明显长于对照组,α角、MA和PLT明显小于对照组(P<0.05)。研究组的DIC、HELLP、心肾功能损伤、胎盘早剥发生率明显高于对照组(P<0.05)。研究组的剖宫产率明显高于对照组,终止妊娠孕周明显短于对照组(P<0.05)。研究组产后出血、子宫切除、新生儿早产、窒息、促肺成熟治疗、低体重儿发生率均明显高于对照组,新生儿体重和Apgar评分均明显低于对照组(P<0.05)。结论高龄瘢痕子宫再次妊娠合并重度子痫前期孕产妇总体凝血功能呈现低凝状态,这增加了母婴并发症发生风险,严重影响妊娠结局。
Objective To explore the effect of re-pregnancy of elderly scarred uterus combined with severe preeclampsia on pregnancy outcome.Methods A total of 65 cases of pregnant women with re-pregnancy of elderly scarred uterus combined with severe preeclampsia who delivered in our hospital were selected as study group.In addition,73 cases of pregnant women with simple re-pregnancy of elderly scarred uterus who delivered in our hospital at the same time were selected as control group.The coagulation function indexes,complications,cesarean section rate,gestational week and pregnancy outcome were compared between the two groups.Results On admission,the R and PT of the study group were significantly longer than those in the control group,and theαangle,MA and PLT in the study group were significantly lower than those in the control group(P<0.05).The incidences of DIC,HELLP,heart and kidney function damage,placental abruption in the study group were significantly higher than those in the control group(P<0.05).The cesarean section rate in the study group was significantly higher than that in the control group,and the gestational week of termination of pregnancy was significantly shorter than that in the control group(P<0.05).The incidences of postpartum hemorrhage,hysterectomy,premature birth,asphyxia,lung ripening therapy and infant of low-birth weight in the study group were significantly higher than those in the control group,and the neonatal weight and Apgar score were significantly lower than those in the control group(P<0.05).Conclusion The overall blood coagulation function of pregnant women with re-pregnancy of elderly scarred uterus combined with severe preeclampsia is hypocoagulable,which increases the risk of maternal and infant complications and seriously affects the pregnancy outcome.
