早发冠心病患者载脂蛋白E基因多态性及与冠脉病变严重程度的关系 被引量:5

The Relationship Between Apolipoprotein E Gene Polymorphism and Severity of Coronary Artery Disease in Patients with Premature Coronary Heart Disease

摘要 目的研究早发冠心病(coronary heart disease,CHD)患者载脂蛋白E(apolipoprotein E,ApoE)基因多态性及与冠脉病变严重程度的关系。方法研究选自2017年9月—2018年9月行冠脉造影术的冠心病患者,按照年龄分组为早发组及迟发组,同期收集健康体检人员为对照组,应用PCR-荧光探针法,测定早发冠心病(100例)、迟发冠心病(110例)和对照组(110例)的ApoE基因型,冠心病患者急诊或择期行冠脉造影术并进行syntax评分。结果早发组高密度脂蛋白均值低于迟发组,甘油三酯均值高于迟发组,具有冠心病家族史所占比例高于迟发组(P<0.05);共检测到5种ApoE基因型,无论是冠心病患者还是对照组人群,等位基因E3频率最高,早发组E4等位基因频率高于迟发组及对照组(20%&15.5%,20%&12.3%,P<0.01),早发组E4/3频率高于迟发组及对照组(30%&25.5%,30%&23.6%,P<0.01);无论是早发组还是迟发组,ApoE4异构体的冠脉syntax>32的比例明显高于ApoE2、ApoE3(12%&7.7%&13.5%,P<0.05;29%&20%&14.5%,P<0.05)。结论E4等位基因不但影响冠心病的发生而且影响冠脉病变的严重程度,且在早发冠心病患者中趋势更明显。 Objective To investigate the relationship between apolipoprotein E(ApoE)gene polymorphism and severity of coronary artery disease in patients with premature coronary heart disease(p-CHD).Methods From September 2017 to September 2018,patients were admitted to emergency department or outpatient department of cardiology in the second affiliated Hospital of Shenyang Medical College and diagnosed as coronary artery disease by coronary angiography.The patients were divided into early onset group and late onset group according to their age.At the same time,the health examiners were collected as the control group.ApoE genotypes were detected by PCR-fluorescence probe me thod in 100 patients diagnosed as p-CHD,110 patients diagnosed as delayed CHD and 110 health checkup people teamed as control group.Emergency or selective coronary angiography and syntax score were performed in CHD patients.Results In p-CHD group,the mean value of high density lipoprotein was lower while the mean value of triglyceride and the proportion of family history of CHD was higher than that in delayed CHD group(P<0.05).We detected five ApoE genotypes.The frequency of E3 allele was the highest both in CHD patients and control group(20%&15.5%,20%&12.3%,P<0.01).The frequency of E4/3 gene in p-CHD group was higher than that in delayed CHD group and control group(30%&25.5%,30%&23.6%,P<0.01).Both in p-CHD group and delayed CHD group,the patients with ApoE4 isomers,the percentage of the coronary artery syntax score more than 32 were significantly higher than that with ApoE2 and ApoE3 isomers(12%&7.7%&13.5%,P<0.05;29%&20%&14.5%,P<0.05).Conclusions E4 allele not only affects the occurrence of coronary heart disease but also affects the severity of coronary artery disease,and the trend is more obvious in p-CHD patients.
