摘要
〔目的〕总结登革热的临床特征和诊断体会,提高登革热的临床诊治技能。〔方法〕回顾性分析佛山市三水区人民医院2019年8月至2019年11月入住感染科的224例登革热患者的临床资料,于发病第1、3、5 d行血常规检查患者的白细胞计数、血小板计数变化规律。〔结果〕224例登革热患者的主要临床表现为发热(100%,224例)、乏力(92%,206例)、肌痛(81.7%,183例)、头痛(74.6%,167例),恶心(56.3%,126例)、畏寒(69.2%,155例)、皮疹(39.7%,89例)、腹泻(24.6%,55例)、呕吐(27.2%,61例)等。丙氨酸氨基转移酶(ALT)升高(43.3%,97例),天门冬氨酸氨基转移酶(AST)升高(69.2%,155例),肌酸激酶(CK)升高(50%,112例),乳酸脱氢酶(LDH)升高(63.4%,142例),尿蛋白阳性(36.2%,81例)。224例患者发病第1、3、5 d外周血白细胞计数减少者所占比例分别为(8%,18例)、(84.4%,189例)、(74.1%,166例);血小板计数减少者所占比例分别为(0%,6例)、(53%,119例)、(72.8%,163例)。〔结论〕三水市2019年登革热患者临床表现以典型登革热症状为主,大部分患者白细胞计数减少开始出现于发病第3 d,血小板计数减少开始出现于发病第5 d;部分患者有肝脏、肾脏和肌肉损害。
〔Objective〕To summarize the clinical characteristics and diagnosis experience of dengue fever,improve the clinical diagnosis and treatment skills of dengue fever.〔Methods〕Retrospective analysis of the clinical data of 224 cases of dengue fever patients admitted to the Department of Infectious Diseases of the Foshan Sanshui District People's Hosptial from August to December of 2019.The routine blood test was taken on the 1st,3rd and 5th days after the onset of the disease,the indexes included white blood count and platelet count change.〔Results〕Among the 224 cases,the common manifestations included fever(100%,224/224),fatigue(92%,206/224),myalgia(81.7%,183/224),headache(74.6%,167/224),disgusting(56.3%,126/224),chilly(69.2%,155/224),skin rash(39.7%,89/224),diarrhea(24.6%,55/224),vomiting(27.2%,61/224),increased alanine aminotransferase(ALT)(43.3%,97/224),elevated aspartate aminotransferase(AST)(69.2%,155/224),increased creatine kinase(CK)(50%,112/224),elevated lactate dehydrogenase(LDH)(63.4%,142/224)and positive urine protein(36.2%,81/224).In the blood testing,8%(18/224)and 0%(0/224)were found with leucopenia and thrombocytopenia respectively by the 1st routine blood testing;84.4%(189/224)and 53%(119/224)were found with leucopenia and thrombocytopenia respectively by the 3rd routine blood testing;74.1%(166/224)and 72.8%(163/224)were found with leucopenia and thrombocytopenia respectively by the 5th routine blood testing.〔Conclusion〕Between August and December of 2019,patients with dengue fever in Sanshui presented the typical clinical characteristics of dengue fever;leukopenia and thrombocytopenia occurred in most of them on the 3rd and 5th days after the onset of the disease respectively;liver and kidney and muscle damage could be found in some of the patients.
