摘要
目的:探讨不同促排卵方案对高龄女性患者卵颗粒细胞凋亡相关基因Caspase 3、Fas和FasL表达的影响。方法:选取甘肃省妇幼保健院生殖医学中心2017年1~12月收治的高龄女性患者194例,按卵巢刺激方案不同分为短效长方案组(n=52)、拮抗剂方案组(n=47)、超长方案组(n=45)和高孕酮状态下促排卵(PPOS)方案组(n=50),比较各组的基础性激素、获卵数、成熟卵率、受精率、优质胚胎率、卵裂率及临床妊娠率,取卵日收集卵泡液,分离提取卵颗粒细胞,采用实时荧光定量PCR(qPCR)检测并比较各组卵颗粒细胞Caspase 3、Fas和FasL mRNA的表达水平。结果:拮抗剂组和PPOS方案组的获卵数、成熟卵率、受精率和优质胚胎率无显著差异(P>0.05),但均显著低于超长方案组(P<0.05),各组患者卵颗粒细胞中均有Caspase3、Fas以及FasL的表达;PPOS方案组、短效长方案组和拮抗剂方案组患者卵颗粒细胞Caspase 3、Fas以及FasL的表达水平均显著高于超长方案组(均P<0.05);短效长方案组和拮抗剂方案组卵颗粒细胞Caspase 3、Fas以及FasL的表达水平比较,差异无统计学意义(P>0.05)。结论:不同促排卵方案对高龄女性患者卵颗粒细胞凋亡相关基因的表达具有不同的影响,高龄不孕患者采用超长方案促排卵可以获得较好的妊娠结局。
Objective: To explore the effects of different ovulation induction programs on the expression of granulosa cell apoptosis-related genes caspase 3, Fas, and FasL in elderly female patients. Methods: 194 elderly female patients admitted to the Reproductive Medicine Center of Gansu Provincial Maternity and Child-care Hospital from January 2017 to December 2017 were selected and divided into the short-acting long protocol group(n=52), antagonist group(n=47), ultra-long protocol group(n=45), and the progestin-primed ovarian stimulation(PPOS) group(n=50) according to their ovarian stimulation scheme. The basic sex hormones, number of eggs obtained, mature egg rate, fertilization rate, high-quality embryo rate, cleavage rate, and clinical pregnancy rate of each group were compared. On the day of egg collection, follicular fluid was collected, and granulosa cells were separated and extracted. Real-time fluorescence quantitative PCR(qPCR) was used to detect and compare the mRNA expression levels of Caspase 3, Fas, and FasL in each group. Results: The retrieved oocytes, mature oocyte rates, fertilization rates and good-embryo rates were no significant differences between GnRH-ant group and PPOS group(P>0.05), but they were significantly lower than the ultra-long group(P<0.05).There were no significant differences in the number of eggs obtained, mature egg rate, fertilization rate, and high-quality embryo rate between the antagonist group and the PPOS group(P>0.05), but they were all significantly lower than those of the ultra-long program group(P<0.05). The expression of caspase 3, Fas, and FasL were detected in the granulosa cells of each group. The expression levels of caspase 3, Fas, and FasL in the granulosa cells of the PPOS group, short-acting long protocol group, and antagonist group were significantly higher than those of the ultralong protocol group(all P<0.05).
