摘要 目的观察血液灌流(HP)联合血液滤过(HF)对急性重度中毒患者血清生化指标的影响。方法选取2018年6月至2019年5月于本院接受治疗的重度急性有机磷农药中毒(AOPP)患者76例,应用随机数字表法分为两组,各38例。对照组接受血液透析(HD)、HP治疗,观察组接受HP、HF治疗。对比两组血清生化指标(白细胞介素-6、转化生长因子-β1)、病死率及死亡病例生存时间。结果治疗前,两组IL-6、TGF-β1水平比较差异无统计学意义;治疗后,观察组IL-6(70.39±15.41)ng/L、TGF-β1(713.69±95.31)ng/L低于对照组(86.74±18.20)ng/L、(996.85±110.27)ng/L,差异有统计学意义(P<0.05);观察组病死率(26.32%)低于对照组(50.00%),生存时间(17.85±5.36)d长于对照组(13.20±4.41)d,差异有统计学意义(P<0.05)。结论重度AOPP患者接受HP、HF联合治疗有利于降低炎性因子水平,延长患者生存时间,提高生存率。 Objective To observe the effect of hemoperfusion(HP) combined with hemofiltration(HF) on the serum biochemical indexes of patients with acute severe poisoning. Methods 76 patients with severe acute organophosphorus pesticide poisoning(AOPP) treated in our hospital from June 2018 to May 2019 were divided into two groups according to random number method, with 38 cases in each group. The control group received hemodialysis(HD) and HP treatment, and the observation group was treated with HP and HF. The serum biochemical parameters [interleukin-6(IL-6)], transformation growth [factor-1(TGF-β 1)], case-fatality rate and death case survival time were compared between two groups.Results There was no significant difference in the level of IL-6,TGF-β 1 between the two groups before treatment, but IL-6(70.39 ±15.41) ng/L,TGF-β 1(713.69 ± 95.31) ng/L in the observation group were significantly lower than that in the control group(86.74 ± 18.20) ng/L,(996.85 ±110.27) ng/L after treatment. The mortality of the observation group(26.32%) was lower than that of the control group(50.00%), and the survival time was(17.85±5.36) d was longer than that of the control group(13.20±4.41) d, and the difference was statistically significant(P<0.05). Conclusion The combination of HP and HF in patients with severe AOPP is beneficial to reducing the level of inflammatory factors, prolonging the survival time of patients and improving the survival rate.
