摘要
目的探讨成人早期股骨头缺血性坏死的MRI检查及影像学表现。方法随机选择该院2018年6月—2019年6月收治的100例成人早期股骨头缺血性坏死的患者作为该次研究入组对象,依次采用CT和MRI进行检查,对纳入的所有患者依次采用CT和MRI进行检查,所有患者采用相同的CT或MR检查仪器进行。检查时嘱咐并协助患者仰卧位,检查后收集患者检查资料并进行对比分析。结果对比2种方法分期诊断结果显示,MRI早期诊断率88.89%及总体诊断率97.24%均高于CT,后者分别为52.78%和86.21%,差异有统计学意义(χ^2=8.24、9.32,P<0.05)。对比2种方法诊断特点显示,CT诊断早期股骨头坏死灵敏度为56.3%,特异度为96.8%,误诊率为3.2%,漏诊率为43.5%。MRI诊断早期股骨头坏死灵敏度为92.3%,特异度为98.8%,误诊率为1.2%,漏诊率为7.6%。组间比较差异有统计学意义(χ^2=6.79,P<0.05)。结论成人早期股骨头缺血性坏死的MRI检查诊断率高,尤其是早期诊断效果较CT更有优势,灵敏度特异度等高,值得临床推广。
Objective To investigate the MRI and imaging findings of early adult ischemic necrosis of the femoral head.Methods 100 patients with early adult femoral head ischemic necrosis treated in the hospital from June 2018 to June 2019 were randomly selected as the subjects of this study.CT and MRI were used in this order.CT and MRI examinations were performed,and all patients were performed using the same CT or MR examination equipment.The patient was instructed and assisted in the supine position during the examination,and the patient examination data was collected and compared for analysis after the examination.Results Comparing the results of staging diagnosis of the two methods,the early diagnosis rate of MRI was 88.89%and the overall diagnosis rate was 97.24%,which were higher than those of CT.The latter were 52.78%and 86.21%,,the difference is statistically significant(χ^2=8.24,9.32,P<0.05).Comparing the diagnostic features of the two methods,the sensitivity of CT for early diagnosis of femoral head necrosis was 56.3%,the specificity was 96.8%,the misdiagnosis rate was 3.2%,and the missed rate was 43.5%.The sensitivity of MRI in the diagnosis of early femoral head necrosis was 92.3%,the specificity was 98.8%,the misdiagnosis rate was 1.2%,and the missed rate was 7.6%.The difference between the groups was statistically significant(χ^2=6.79,P<0.05).Conclusion The early diagnosis of ischemic necrosis of the femoral head in adults is highly diagnosed by MRI,especially the early diagnosis is more advantageous than CT,and the sensitivity and specificity are high,which deserves widespread clinical promotion.
