摘要
目的:研究单孔腹腔镜手术对卵巢良性肿瘤患者免疫功能的改善效果。方法:选取2017年4月-2018年8月本院收治的卵巢良性肿瘤患者102例,分为对照组(传统多孔腹腔镜手术)和观察组(单孔腹腔镜手术),比较两组临床疗效。结果:两组手术过程顺利,无中转开腹的病例,术中无肠损伤发生,术后无切口感染、切口疝等并发症发生。观察组术后排气时间(13.6±1.9)h、住院时间(2.7±1.3)d短于对照组,手术时间(68.7±8.6)min长于对照组,但术后血红蛋白下降幅度(12.8±3.1g/L)与对照组比较无差异(P>0.05)。两组术前VAS评分比较无差异(P>0.05),观察组术后24h、48h、72h时VAS评分(2.6±1.3)分、(1.9±1.0)分、(1.1±0.6)分均低于对照组(P<0.05)。两组术前免疫功能指标比较无差异(P>0.05),观察组术后24h、72h时CD3+、CD4+和CD4+/CD8+水平高于对照组,CD8+水平低于对照组(P<0.05)。结论:单孔腹腔镜手术应用于卵巢良性肿瘤对机体免疫功能影响较小,安全有效。
Objective:To study the effect of improving immune function of single hole laparoscopic surgery for treating patients with benign ovarian tumors.Methods:102 patients with benign ovarian tumors were divided into two groups from April 2017 to August 2018.The patients in the control group were given traditional porous laparoscopic surgery and the patients in the observation group were given single hole laparoscopic surgery.The clinical efficacy of patients was compared between the two groups.Results:In the two groups,the operations of all patients were all smooth,and the conversion to laparotomy of patient had not occurred.The complications,such as intestinal injury during the operation,postoperation incision infection and incisional hernia,and other complications of patient in both groups hadn’t occurred.The postoperative exhaust time,hospitalization time and operation time of patients in the observation group were 13.6±1.9 h,2.7±1.3 d,and 68.7±8.6 min,respectively,which were significant longer than those of patients in the control group,However,the degree of Hb level decreased of patients in the observation group was 12.8±3.1g/L,which had no significant different to that of patients in the control group(P>0.05).There was no significant different in VAS score of patients before operation between the two groups(P>0.05).The VAS score of patients in the observation group at 24 h,48 h and 72 h after operation were 2.6±1.3 points,1.9±1.0 points,and 1.1±0.6 points,respectively,which were significant lower than those of patients in the control group.There was no significant different in preoperative immune function of patients between the two groups(P>0.05).The levels of CD3+,CD4+and CD4+/CD8+of patients in the observation group at 24 h and 72 h after operation were significant higher than those of patients in the control group,but the level of CD8+of patients in the observation group was significant lower(P<0.05).Conclusion:Single hole laparoscopic surgery for treating patients with benign ovarian tumors has less
