摘要
目的探讨不同运动靶心率评价方法在慢性心力衰竭(CHF)患者运动康复中的临床价值。方法选择2018年5月—2019年10月在石家庄市第三医院就诊的CHF患者84例作为研究对象,均采用心肺运动试验(CPET)个性化康复运动方案,记录无氧阈、峰值摄氧量、峰值氧脉搏。比较无氧阈法、峰值摄氧量法、心率储备法及年龄预计法计算运动靶心率水平。结果年龄预计法计算运动靶心率与无氧阈法比较,差异有统计学意义(P<0.05);峰值摄氧量法及心率储备法计算运动靶心率与无氧阈法比较,差异无统计学意义(P>0.05)。峰值摄氧量法和心率储备法计算运动靶心率与无氧阈法计算运动靶心率呈正相关(r值分别为0.87、0.75,P<0.05);而年龄预计法计算运动靶心率与无氧阈法计算运动靶心率无直线相关关系(r=-0.14,P=0.54)。结论CHF患者中,采用心率储备法和峰值摄氧量法计算的运动靶心率与无氧阈法接近,具有临床推广使用价值。
Objective To investigate the clinical value of different evaluation methods for target heart rate of exercise rehabilitation in patients with chronic heart failure(CHF).Methods 84 patients with CHF in Shijiazhuang Third Hospital from May 2018 to October 2019 were selected as the research object.The cardiopulmonary exercise test(CPET)was used to individual rehabilitation exercise program,the anaerobic threshold,peak oxygen uptake and peak oxygen pulse were recorded.The target heart rate for which determined based on anaerobic threshold,peak oxygen uptake,heart rate reserve,age method were compared.Results There was significant difference in target heart rate determined by age method and anaerobic threshold method(P<0.05).There was no significant difference in target heart rate determined by peak oxygen uptake method,heart rate reserve method and anaerobic threshold method(P>0.05).The target heart rate determined by peak oxygen uptake method,heart rate reserve method was positively correlated with that determined by anaerobic threshold method,respectively(r value was 0.87,0.75,P<0.05).The target heart rate determined by age method was no linear correlated with that determined by anaerobic threshold method(r=-0.14,P=0.54).Conclusion The evaluating value of heart rate reserve method and peak oxygen uptake method in target heart rate of aerobic rehabilitation exercise of patients with CHF should be close to anaerobic threshold method,which possess clinical application value.
