摘要
目的探讨关节镜下自体骨软骨移植镶嵌术(OAT)治疗股骨髁负重区软骨磨损的疗效。方法选取2017年9月~2019年3月因股骨髁软骨磨损于潍坊市人民医院就诊,并行关节镜下自体骨软骨移植镶嵌术的患者23例为研究对象,比较术前及术后1年患者膝关节疼痛视觉模拟评分,Lysholm膝关节评分以及IKDC评分变化。结果术后所有观察对象刀口均愈合良好,无膝关节感染、下肢深静脉血栓等严重并发症发生。23例患者平均VAS评分由术前(6.30±1.26)分降至(2.74±0.92)分,差异有显著性(P<0.05)。Lysholm膝关节评分由术前(69.83±5.45)分提高至术后(91.13±5.05)分,差异有显著性(P<0.05)。膝关节IKDC评分由术前(73.61±3.04)分变为术后(93.26±2.24)分,差异有显著性(P<0.05)。结论关节镜下自体骨软骨移植镶嵌技术治疗股骨髁负重区软骨磨损,能够改善患者膝关节疼痛,提高患者Lysholm膝关节评分以及膝关节IKDC评分,疗效可靠,值得临床推广应用。
Objective To present the clinical outcomes of arthroscopic osteochondral autograft transfer of the weight-bearing area of femoral condyle.Methods Twenty-three patients with osteochondral lesions of the weight-bearing area of femoral condyle underwent arthroscopic osteochondral autograft transfer in Weifang People’s Hospital from 2017-9 to 2019-3 were selected.Clinical outcomes were evaluated preoperatively and postoperatively according to compare the International Knee Documentation Committee score,the Visual Analogue Scale of pain,the Lysholm knee scole.Results All the wounds well recovered,and no serious complications such as knee joint infection and lower limb deep vein thrombosis occurred.The average VAS score of 23 patients decreased from 6.30±1.26 points before operation to 2.74±0.92 points,which has significant difference(P<0.05).The Lysholm scole was increased from 69.83±5.45 to 91.13±5.05,P<0.05,the difference was statistically significant.The IKDC scole was increased from 73.61±3.04 to 93.26±2.24,P<0.05,the difference was statistically significant.Conclusion Arthroscopic autologous cartilage transplantation mosaic technique can relief the knee pain,improve Lysholm knee score and knee IKDC score,which has convincing effect,and it is worthy of clinical application.
