摘要
目的探讨家庭医生签约服务对高血压患者血压及其危险因素的控制效果。方法选取2018年1月-2019年1月在东莞市樟木头镇社区卫生服务中心签约家庭医生服务的200例高血压患者为研究对象,建立签约电子档案,根据临床症状、体征、辅助检查结果与生活习惯以及心理、社会因素制订个性化的综合管理措施。比较管理前、后,血压水平(舒张压、收缩压)、空腹血糖、BMI、血脂[总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)]、内生肌酐清除率(Ccr)和生活方式情况。结果管理后舒张压、收缩压、空腹血糖、BMI、TG、TC、LDL-C水平均低于管理前,HDL-C、Ccr水平高于管理前,差异均有统计学意义(P<0.05)。管理后饮酒、吸烟比例低于管理前,规律服药、适量运动、摄盐情况(≤6g)比例高于管理前,差异均有统计学意义(P<0.05)。结论家庭医生签约服务可有效控制高血压患者危险因素和血压水平,改善生活方式,有利于提高患者生活质量,具有较高的临床应用价值。
Objective To explore the control effect of family doctor contract services on blood pressure and its risk factors in patients with hypertension.Methods From January 2018 to January 2019,200 patients with hypertension who signed a contract with family doctor in Community Health Service Center of Zhangmutou Town of Dongguan City were selected as the research objects to establish a contract electronic file and formulate personalized comprehensive management measures according to clinical symptoms,signs,auxiliary examination results,living habits,psychological and social factors.Blood pressure(diastolic pressure,systolic pressure),fasting blood glucose,BMI,blood lipids(total cholesterol(TC),LDL cholesterol(LDL-C),triylglycerides(TG),HDL-C),endocreatin creatinine clearance rate(Ccr),lifestyle conditions were compared before and after management.Results The levels of diastolic blood pressure,systolic blood pressure,fasting blood glucose,BMI,TG,TC and LDL-C after management were lower than those before management,and the levels of HDL-C and Ccr were significantly higher than those before management,and the differences were statistically significant(P<0.05).The proportion of drinking and smoking after management were lower than those before management,the proportion of regular medication,moderate exercise and salt intake(≤6 g)were significantly higher than those before management,and the differences were statistically significant(P<0.05).Conclusion Family doctor contract service could effectively control risk factors and blood pressure level of patients with hypertension,improve patients’life style and quality of life,and had high clinical application value.
