摘要
目的研究对非杓型伴清晨高血压患者行不同服药时间替米沙坦治疗的效果。方法选取2018年4月至2019年4月间于我院治疗的非杓型伴清晨高血压患者104例,按简单随机化法分为对照组和观察组,各52例。两组均行替米沙坦治疗,对照组于清晨服药,观察组于夜间服药。比较两组治疗后24h动态血压、血管内皮功能及左心室质量指数(LVMI)的情况。结果治疗3个月后,两组全天平均舒张压、白天平均舒张压及夜间平均舒张压比较无统计学意义(P>0.05),观察组全天平均收缩压、白天平均收缩压及夜间平均收缩压低于对照组(P<0.05);观察组血管性假血友病因子(vWF)水平低于对照组,内皮依赖性舒张反应(EDD)及一氧化氮(NO)水平高于对照组(P<0.05);观察组LVMI低于对照组(P<0.05)。结论替米沙坦夜间服用可有效降低患者动态收缩压水平,改善患者血管内皮功能和左心室功能。
Objective To study the effect of telmisartan treatment in non type patients with early morning hypertension with different medication times.Methods A total of 104 patients with non-diarrheal type and early morning hypertension who were treated in our hospital from April 2018 to April 2019 were selected and divided into a control group and an observation group by simple randomization,each with 52 cases.Both groups were treated with telmisartan,the control group was administered in the morning and the observation group was administered at night.The 24 h post-treatment ambulatory blood pressure,vascular endothelial function,and left ventricular mass index(LVMI)were compared between the two groups.Results After 3 months of treatment,the average diastolic blood pressure,daytime diastolic blood pressure,and nighttime diastolic blood pressure in the two groups were not statistically significant(P>0.05).The observation group had average daily systolic blood pressure,daytime systolic blood pressure,and nighttime average blood pressure.The systolic blood pressure was lower than that of the control group(P<0.05);the von Willebrand factor(vWF)levels in the observation group were lower than those in the control group,and the levels of endothelial-dependent diastolic response(EDD)and nitric oxide(NO)were higher than those in the control group(P<0.05);LVMI in observation group was lower than that in control group(P<0.05).Conclusion Taking telmisartan at night can effectively reduce the level of dynamic systolic blood pressure and improve vascular endothelial function and left ventricular function in patients.
