摘要
目的探讨失效模式和效应分析在降低门诊手术病理标本安全风险管理中的效果。方法成立门诊手术室失效模式和效应分析小组,以“门诊手术病理标本安全风险管理”为主题,依据流程分析、计算风险评估指数(RAC)和风险优先数(RPN)、评估结果、计划改善等程序,分析改善前3个月门诊手术病理标本管理中存在的问题及原因,制订相应的对策并组织实施,再次进行效应分析。结果实施失效模式和效应分析后3个月,门诊手术病理标本管理问题发生率从7.28%下降为2.25%。结论失效模式和效应分析在降低门诊手术病理标本管理问题发生率方面的效果显著,值得应用推广。
Objective To explore the effect of failure mode and effects analysis(FMEA)in reducing the safety risk of outpatient surgery pathology samples.Methods Outpatient operating room FMEA team was established,with the theme of outpatient surgery pathology sample safety risk management,to analyze the problems and causes of outpatient surgery pathology sample management of three months before improvement.The analysis included procedure analysis,Risk Assessment Code and Risk Priority number computation,result evaluation and improvement planning.Countermeasures were decided and carried out,and then FMEA was conducted again.Results At 3 months after the implementation of FMEA,the defect rate of outpatient surgery pathology sample management decreased to 2.25%from 7.28%.Conclusion FMEA is effective in reducing the defect rate of outpatient surgery pathology sample management and is worthy of popularization.
