肥胖所致脂肪胰及其对2型糖尿病的影响 被引量:9

Obesity-induced fatty pancreas and its effects on type 2 diabetes mellitus

摘要 肥胖可致机体多余脂肪异位沉积在非脂肪组织如肝脏、心脏、肌肉、肾脏和胰腺等,并通过脂毒性损害靶器官功能。2011年Smits等将胰腺异位脂肪沉积命名为脂肪胰,其可表现为脂肪替代或脂肪浸润,后者是肥胖所致脂肪胰的主要表现形式。脂肪胰可能通过慢性炎性反应、内质网应激和瘦素抵抗等多种途径引起β细胞分泌功能受损和胰岛素抵抗,最终导致2型糖尿病的发生或进展。目前,MRI为检测胰脏脂肪含量的首选方法,但尚缺少公认的疾病诊断和分级标准。已有研究表明,控制饮食、增加运动、减轻体重以及合理用药可减轻肥胖所致的胰腺脂肪沉积。 Obesity leads to the ectopic deposition of excess fat in the non-adipose tissues such as liver,heart,muscle,kidney and pancreas,and damages the function of target organ through lipotoxicity.In 2011,Smits et al.named ectopic fat deposition in the pancreas as fatty pancreas,which is manifested as fat replacement or fat infiltration.Fat infiltration is the main cause of obesity-induced fatty pancreas.During the course of fatty pancreas,chronic inflammatory response,endoplasmic reticulum stress,leptin resistance and other pathways cause impaired secretion function ofβcells and insulin resistance,and eventually lead to the occurrence and progression of type 2 diabetes mellitus.MRI is the preferred method to detect pancreatic fat content,but there is no recognized diagnostic and grading criteria.Studies have shown that diet control,exercise,weight loss and drugs can reduce obesity-induced fat deposition in the pancreas.
