摘要
目的观察磁共振弥散加权成像(MR-DWI)在卵巢癌定性、分期诊断中的应用价值。方法回顾性分析2017年3月至2019年3月南阳市中心医院128例行手术治疗卵巢肿瘤患者的临床资料,均行MR-DWI检查,观察MR-DWI在卵巢癌定性、分期诊断中的应用价值。结果 MR-DWI诊断卵巢癌的准确度(91.41%)、灵敏度(84.62%)、特异度(96.05%),Kappa值0.819,一致性较强。不同分期卵巢癌患者及卵巢良性肿瘤患者ADC值差异有统计学意义(P<0.05)。ADC值诊断Ⅰ、Ⅱ、Ⅲ、Ⅳ期卵巢癌的AUC分别为0.973、0.976、0938、0.986(P<0.05),灵敏度分别为0.962、0.909、1.000、1.000,特异度分别为0.908、0.940、0.845、0.975,临界值分别为1.10、0.97、0.90、0.73×10-3mm2/s。结论 MR-DWI在卵巢癌定性、分期诊断中有较高应用价值。
Objective To observe the application value of magnetic resonance diffusion weighted imaging (MR-DWI) in the qualitative and staging diagnosis of ovarian cancer.Methods The clinical data of 128 patients with ovarian tumors undergoing surgical treatment from March 2017 to March 2019 were retrospectively analyzed.MR-DWI was performed,and the application value of MR-DWI in the qualitative and staging diagnosis of ovarian cancer was observed.Results The accuracy,sensitivity,specificity and Kappa value of MR-DWI in diagnosing ovarian cancer were 91.41%,84.62%,96.05% and 0.819,with high consistency.There were significant differences in the ADC values among patients with different stages of ovarian cancer and those with benign ovarian tumors (P<0.05).The AUC values of ADC in the diagnosis of stage Ⅰ,Ⅱ,Ⅲ,and Ⅳ ovarian cancer were 0.973,0.976,0938,and 0.986 respectively (P<0.05),and the sensitivities were 0.962,0.909,1.000 and 1.000,and the specificities were 0.908,0.940,0.845 and 0.975 respectively,and the critical values were 1.10,0.97,0.90 and 0.73×10-3mm2/s.Conclusion MR-DWI has a high application value in the qualitative and staging diagnosis of ovarian cancer.
