摘要
目的探讨分析血压精准控制对颈动脉内膜剥脱围术期并发症的影响。方法选取2018年7月~2019年6月在惠州市第一人民医院神经外科进行诊治的颈动脉粥样硬化斑块患者40例作为研究对象,所有患者均接受颈动脉内膜剥脱手术,并术后3~6个月内对其进行复查,观察参加本实验患者的术前血压变化,精准控制术中血压、术后血压的范围。通过及时调整与综合防范措施,减少患者出现围术期并发症。结果参加本实验的38例患者的切口愈合等级为甲,1例切口愈合等级为乙,1例切口愈合等级为丙。2例患者出现脑高灌注综合征症状,1例患者出现脑梗死,2例患者出现术后切口感染,经治疗后所有研究对象全部康复出院。结论颈动脉内膜剥脱围术期并发症重点在预防,仔细的术前准备、术中、术后血压的积极干预能有效的降低并发症的发生,随着相关技术的日益完善,值得在临床中广泛推广应用。
Objective To explore and analyze the effect of precise control of blood pressure(BP)on the perioperative complications of carotid endarterectomy(CEA).Methods 40 patients with carotid atherosclerotic plaque diagnosed and treated in Department of Neurosurgery,Huizhou First Hospital were selected as the subjects of study.The study lasted from July 2018 to June 2019.All the patients underwent CEA and were re-examined 3 to 6 m after operation.The preoperative BP changes of the patients participating in the experiment were observed,and the range of intraoperative BP and postoperative BP was precisely controlled.Through timely adjustment and comprehensive preventive measures,perioperative complications were reduced for the patients.Results The grade of incision healing of 38 patients participating in this experiment was A,that of 1 patient was B,and that of 1 patient was C.2 patients had symptoms of cerebral hyperperfusion syndrome,1 patient suffered cerebral infarction,and 2 patients had postoperative incision infection.After treatment,all the subjects of study recovered and were discharged from hospital.Conclusion The key point for the perioperative complications of CEA is prevention.Careful preoperative preparation and active intervention on intraoperative and postoperative BP can effectively reduce the occurrence of complications.With the gradual amelioration of the related technologies,it is worthy of extensive promotion and application in clinical practice.
