摘要
目的探讨外周血淋巴细胞亚群变化在肝移植术后患者早期感染中的意义。方法采集2019年5—8月在青岛大学附属医院行肝脏移植手术的38例患者的外周血共76例次,根据是否发生感染,分为非感染组和感染组。感染组患者采集感染时和感染治愈后的全血样本各19例次,非感染组患者采集同时间的全血样本各19例次。应用流式细胞术检测样本中CD4^+T细胞、CD8^+T细胞、CD19^+B细胞、CD3-CD56^+NK细胞、CD3^+CD25^+T细胞的百分比和绝对计数,并计算CD4^+/CD8^+比值。结果感染时感染组血清中CD4^+T细胞及CD8^+T细胞的百分比显著低于非感染组(t=2.192、2.316,P<0.05);血清中CD19^+B细胞百分比则显著高于非感染组(t=-2.212,P<0.05);两组间血清中CD3-CD56^+NK细胞、CD3^+CD25^+T细胞的百分比以及CD4^+/CD8^+比值比较差异无显著性(P>0.05)。而感染治愈后两组间上述指标比较差异无显著性(P>0.05)。感染时感染组血清中CD4^+T细胞、CD8^+T细胞、CD3-CD56^+NK细胞和CD3^+CD25^+T细胞的绝对计数均显著低于非感染组(t=2.087~4.136,P<0.05);两组间血清中CD19^+B细胞绝对计数比较差异无显著性(P>0.05)。而感染治愈后两组间上述指标比较差异无显著性(P>0.05)。结论监测肝移植术后早期患者血清中淋巴细胞亚群百分比和绝对计数,有助于评估患者的感染风险。
Objective To investigate the significance of peripheral blood lymphocyte subsets in early infection after liver transplantation.Methods A total of 76 peripheral blood samples were collected from 38 patients who underwent liver transplantation in The Affiliated Hospital of Qingdao University from May to August,2019,and according to the presence or absence of infection,they were divided into non-infection group and infection group.A total of 19 whole blood samples each were collected from the patients in the infection group at the time of infection and after the infection was cured,and 19 whole blood samples each were collected from the patients in the non-infection group at the two time points.Flow cytometry was used to measure the percentages and absolute counts of CD4^+T cells,CD8^+T cells,CD19^+B cells,CD3-CD56^+NK cells,and CD3^+CD25^+T cells,and the CD4^+/CD8^+ratio was calculated.Results Compared with the non-infection group at the time of infection,the infection group had significantly lower percentages of CD4^+T cells and CD8^+T cells in serum(t=2.192,2.316,P<0.05)and a significantly hig-her percentage of CD19^+B cells in serum(t=-2.212,P<0.05),while there were no significant differences between the two groups in the percentages of CD3-CD56^+NK cells and CD3^+CD25^+T cells in serum and CD4^+/CD8^+ratio(P>0.05).After the infection was cured,there were no significant differences in the above indices between the two groups(P>0.05).Compared with the non-infection group at the time of infection,the infection group had significantly lower absolute counts of CD4^+T cells,CD8^+T cells,CD3-CD56^+NK cells,and CD3^+CD25^+T cells in serum(t=2.087-4.136,P<0.05),while there was no significant difference in the absolute count of CD19^+B cells in serum(P>0.05).After the infection was cured,there were no significant differences in the above indices between the two groups(P>0.05).Conclusion Monitoring of the percentages and absolute counts of lymphocyte subsets in serum in the early stage after liver transplantation can hel
