中低强度华法林抗凝的肺血栓栓塞症患者的国际标准化比值与凝血因子Ⅱ和Ⅹ活性的关系 被引量:5

Study on the correlation between INR and the activity of coagulation factorⅡand X in patients with pulmonary thromboembolism with moderate and low intensity anticoagulation by oral warfarin

摘要 目的研究肺血栓栓塞症患者口服华法林在中低强度抗凝时国际标准化比值(INR)和凝血因子Ⅱ与Ⅹ活性水平的相关性。方法51例肺血栓栓塞症规律口服华法林抗凝治疗患者,根据其监测的INR指标分为低强度抗凝组(INR 1.6~2.0)和标准强度抗凝组(INR 2.0~3.0)。测定凝血因子Ⅱ和凝血因子Ⅹ活性水平,分析INR水平和凝血因子活性的相关性。结果低强度抗凝组和标准强度抗凝组INR分别为1.69±0.28和2.55±0.46,对应的凝血因子Ⅱ活性分别为(48.3±28.0)%和(24.0±8.0)%,凝血因子Ⅹ活性分别为(32.8±24.0)%和(16.7±6.0)%。INR和凝血因子Ⅱ和Ⅹ的活性成呈负相关,相关系数分别为–0.903和–0.459。将凝血因子Ⅱ活性<40%,凝血因子Ⅹ活性抑制水平<25%定义为抗凝有效,凝血因子Ⅱ活性水平达到抗凝治疗效果时其所对应的最小INR值为1.56,凝血因子Ⅹ为1.66。结论INR同其凝血因子Ⅱ和凝血因子Ⅹ活性负相关。随着INR的增高,凝血因子Ⅱ和凝血因子Ⅹ活性降低,低强度抗凝无法有效抑制凝血因子活性。 Objective To study the correlation between international normalized ratio(INR)and coagulation factorⅡandⅩin patients with pulmonary thromboembolism treated with warfarin at moderate and low intensity anticoagulation.Methods Fifty-one patients with pulmonary thromboembolism treated with warfarin orally were divided into low-intensity anticoagulation group(INR from 1.6 to 2.0)and standard-intensity anticoagulation group(INR form 2.0 to 3.0)according to their monitoring INR indices.The levels of coagulation factorⅡandⅩwere measured,and the correlation between INR level and coagulation factor activity was compared.Results The INR of the low intensity anticoagulation group was 1.69±0.2 and the standard intensity anticoagulation group was 2.55±0.46.The corresponding activity of coagulation factorⅡwas(48.3±28.0)%and(24.0±8.0)%respectively.The activity of coagulation factorⅩwas(32.8±24.0)%and(16.7±6.0)%.There was a negative correlation between the activity of INR and coagulation factorⅡandⅩ,with correlation coefficients of–0.903 and–0.459,respectively.Coagulation factorⅡactivity<40%,coagulation factorⅩactivity inhibitory level<25%is defined as anticoagulation effect.When coagulation factorⅡactivity level reaches anticoagulation effect,the corresponding minimum INR value was 1.56 and as to coagulation factorⅩ,the corresponding minimum INR value was 1.66.Conclusions INR is negatively correlated with the activity of coagulation factorⅡand coagulation factorⅩ.With the increase of INR,the activity of coagulation factorⅡand coagulation factorⅩdecrease.Low intensity anticoagulation could not effectively inhibit the activity of coagulation factor.
