摘要
目的探讨腹腔镜行子宫腹壁悬吊术联合骶韧带缩短缝合术在子宫脱垂中的应用价值。方法选取2018年1月至2019年4月因子宫脱垂到玉林市妇女儿童医院采用腹腔镜行子宫腹壁悬吊术联合骶韧带缩短缝合术治疗的60例患者作为观察组,另选取2016年1月至2019年4月采用腹腔镜子宫腹壁悬吊术治疗的60例患者为对照组。比较两组的并发症发生率,疗效及患者满意度、性生活质量及心理状况。结果观察组急性并发症发生率8.33%,低于对照组的21.67%;观察组有效率98.33%,高于对照组的91.67%;观察组的满意度100%,高于对照组的90.00%;观察组术后的性生活质量明显改善,术后3、5、7个月的PISQ-12评分均高于对照组;观察组治疗后的汉密尔顿焦虑评分低于对照组;以上各项指标两组比较差异均有统计学意义(P<0.05)。两组不同年龄段的并发症发生情况、疗效比较,差异无统计学意义(P>0.05)。结论腹腔镜行子宫腹壁悬吊术联合骶韧带缩短缝合术治疗子宫脱垂疗效显著,可减少并发症的发生,改善患者的性生活质量及心理焦虑程度,提高患者满意度。
Objective To explore the application value of laparoscopic uterine abdominal wall suspension combined with patellar ligament shortening suture in uterine prolapse.Methods 60 patients with uterine prolapse treated with laparoscopic uterine abdominal wall suspension combined with patellar ligament shortening suture in Yulin Women’s And Children’s Hospital from January 2018 to April2019 were selected as the observation group,and 60 cases treated with laparoscopic uterine abdominal wall suspension from January2016 to April 2019 were the control group.The incidence of complications,efficacy,patient satisfaction,quality of sexual life and psychological status were compared between the two groups.Results The incidence of acute complications in the observation group was8.33 %,which was lower than 21.67 % in the control group;the effective rate in the observation group was 98.33 %,which was higher than 91.67 % in the control group;the satisfaction rate in the observation group was 100 %,which was higher than 90.00 %in the control group;the quality of sexual life of the observation group was significantly improved after operation.The PISQ-12 scores at3,5,and 7 months after the operation were higher than the control group;the Hamilton Anxiety Score of the observation group after treatment was lower than that of the control group;the differences of the above indicators between the two groups were statistically significant(P<0.05).There was no statistically significant differente in the occurrence of complications and efficacy between the two groups at different ages.Conclusion Laparoscopic uterine abdominal wall suspension combined with sacral ligament shortening suture has a significant effect on the treatment of uterine prolapse,which can reduce the occurrence of complications,improve patients’ sexual quality of life and psychological anxiety,and improve patient satisfaction.
