乳腺癌诊断中应用常规超声、超声弹性成像及超声增强造影的价值分析 被引量:15

Analysis of the Value of Conventional Ultrasound,Ultrasonic Elastic Imaging and Ultrasound Enhancement in Breast Cancer Diagnosis

摘要 目的分析常规超声(US)、超声弹性成像(UE)与超声增强造影(CEUS)在乳腺癌诊断中的应用价值。方法随机选取120例乳腺肿块诊治的患者,所有患者均接受US、UE以及CEUS检查,分析病理结果,对比分析US、CEUS声像图特征及UE评分,比较病理检查结果与US、UE以及CEUS诊断价值。结果US声像图特征中良性肿瘤血流信号2~3级、后方回声衰减、钙化、边界模糊、形态不规则占比均明显低于恶性肿瘤,纵横比小于1占比明显高于恶性肿瘤,差异均有统计学意义(P<0.05)。良恶性肿瘤增强特征、增强边界以及增强形态占比差异均有统计学意义(P<0.05)。US、UE及CEUS诊断准确率、漏诊率、误诊率差异均无统计学意义(P>0.05),US诊断特异度为84.21%、灵敏度为77.19%,UE诊断特异度为81.58%、灵敏度为61.40%,CEUS诊断特异度为75%、灵敏度为75.44%,US+UE+CEUS诊断特异度为93.42%、灵敏度为94.74%US+UE+CEUS联合检查诊断准确率特、异度以及灵敏度明显高于单项检查,差异有统计学意义(P<0.05)。结论在乳腺癌诊断中的应用US、UE、CEUS联合诊断能够使诊断准确率、特异度及灵敏度得到显著提高,有助于为临床医生病情诊断以及预后评估提供参考。 Objective To analyze the value of conventional ultrasound(US),ultrasonic elastography(UE)and CEUS in breast cancer diagnosis.Methods 120 patients were selected for breast mass diagnosis and treatment.All patients were examined by US,UE and CEUS.The pathological results were analyzed and the US and CEUS sonogram features and UE scores were compared.Comparison of pathological findings with US,UE and CEUS diagnostic value.Results The blood flow signal of benign tumor in the US sonogram was grade 2-3,the echo attenuation,calcification,blurred boundary,irregular morphology were all significantly lower than the malignant tumor,and the aspect ratio was less than 1,which was significantly higher than the malignant tumor.The difference was statistically significant(P<0.05).The enhancement characteristics of benign malignant tumors,the enhancement boundary,and the proportion of enhanced morphology were statistically significant(P<0.05).The diagnostic accuracy,missed diagnosis and misdiagnosis rates of US,UE and CEUS were not statistically significant(P>0.05),US diagnosis specificity was 84.21%,sensitivity was 77.19%,UE diagnosis specificity was 81.58%,sensitivity was 61.40%,CEUS diagnosis specificity was 75%,sensitivity was 75.44%.US+UE+CEUS diagnostic specificity was 93.42%,sensitivity was 94.74%US+UE+CEUS diagnostic accuracy,specificity,and sensitivity were significantly higher than the individual examination,and the difference was statistically significant(P<0.05).Conclusion The application of US,UE and CEUS in the diagnosis of breast cancer can significantly improve the diagnostic accuracy,specificity and sensitivity,and can provide reference for clinician diagnosis and prognosis evaluation.
