摘要
目的探讨与普通病例相比,重症手足口病的发病是否取决于患者居住于农村、散居、患者为男性、年龄≤3岁等人群特征因素,找出重症手足口病发病的人群特征规律,为重症手足口病易发人群的预防及保护提供参考。方法利用Review Manager 5.3分析软件对我国不同地区调查的2008~2016年间重症手足口病患者人群特征因素(居住于农村、散居、患者为男性、年龄≤3岁)进行Mantel-Haenszel法二分类变量的Meta分析。结果共纳入符合标准的21篇文献,累计调查307713人,其中重症病例8370人,普通病例299343人。Meta分析显示,患者居住于农村(OR=1.18,95%CI:1.05~1.33)、散居(OR=1.55,95%CI:1.29~1.87)、患者为男性(OR=1.13,95%CI:1.07~1.20)、年龄≤3岁(OR=5.18,95%CI:4.33~6.19)均是重症手足口病发病的影响因素。结论患者居住于农村、散居、患者为男性、年龄≤3岁为重症手足口病发病的影响因素,具有以上特征的人群应成为下一次手足口病大流行时的重点保护对象,以降低重症手足口病的发病率及死亡率。
Objective To investigate whether the incidence of severe hand,foot and mouth disease(HFMD)depends on the population characteristics of patients,such as living in rural area,living scattered,male,age of not more than 3 years,and to find the characteristic regularity of population with severe HFMD so as to provide a reference for prevention and protection of susceptible population.Methods Meta analysis was performed by Mantel-Haenszekl method using Review Manager 5.3 software on the characteristic factors of population susceptible to severe HFMD reported in different regions of China from 2008 to 2016,such as living in rural area,living scattered,male and age of not more than 3 years.Results A total of 21 literatures were reviewed,in which 307713 cases of HFMD were involved,including 8370 severe cases and 299343 common cases.Meta analysis showed that living in rural area(OR=1.18,95%CI:1.05~1.33),living scattered(OR=1.55,95%CI:1.29~1.87),male(OR=1.13,95%CI:1.07~1.20)and age of not more than 3 years(OR=5.18,95%CI:4.33~6.19)were influencing factors of the onset of severe HFMD.Conclusion The influencing factors of severe HFMD include living in rural area,living scattered,male and age of not more than 3 years.The population with the above-mentioned characteristics should be considered as the key protection object in the next HFMD pandemic so as to decrease the morbidity and mortality of severe HFMD.
